HelloPharmacist Scorecard of Neem by Herbal Hills
Our pharmacy team’s full take, with four database checks built into the cards below — a summary of what is known, not a grade of the product itself.
What’s inside
Full disclosure
This product contains one active ingredient: a blend of neem leaves extract and neem leaves powder. Neem is an herbal remedy with a long history in traditional medicine, sourced from the neem tree.
The capsule itself is made from HPMC (a plant-based capsule material) and silicon dioxide, both inactive ingredients.
Does it work?
Moderate evidence
The evidence supports neem leaf for gingivitis (gum inflammation) and lice — both are rated possibly effective based on available research. Dental plaque also has a possibly effective rating.
For acne and tungiasis (a parasitic skin condition), the evidence isn't strong enough to say whether it works. We don't hold data on how well this product performs for any other uses.
How safe is it?
Well-documented data
Neem leaf preparations are generally well tolerated when taken by mouth for short periods in adults, though high-quality safety studies haven't been done. Topically (on the skin), neem is well tolerated in both children and adults, though sensitive individuals may get contact dermatitis.
The serious caution: neem oil and seed extracts can be toxic if swallowed, and children are at particular risk of serious harm from oral neem oil. The safety data advises against neem during pregnancy — it may affect fertility and pregnancy outcomes.
There isn't enough reliable safety information to know whether it's okay while breastfeeding, so that's another time to skip it. Rare but serious effects reported with oral neem include heart rhythm problems (cardiac arrest, ventricular fibrillation) and kidney damage in adults, and hemolytic anemia (where red blood cells break down) in people with a glucose-6-phosphate dehydrogenase deficiency.
Talk with your doctor or pharmacist before starting neem if you have any of these concerns or if you're pregnant or nursing.
Meds to double-check
Moderate interaction found
Before taking neem, double-check the following medication types with your doctor or pharmacist: antidiabetes drugs (risk of dangerously low blood sugar), immunosuppressants (may become less effective), and any drugs cleared by your liver or kidneys, including those processed by CYP1A2, CYP2C8, CYP2C9, and CYP3A4 enzymes or P-glycoprotein transport.
The bottom line
Scorecard at a glanceFully disclosed formula with some supporting evidence behind its ingredients' uses. Moderate medication interactions have been identified, and safety information is well characterized.
Neem may help with gingivitis, plaque, and lice based on available evidence. It interacts with over 1,000 medications across several drug-processing pathways, so if you take any prescription drugs — especially diabetes medications, immunosuppressants, or anything your liver breaks down — you need to check with your own doctor or pharmacist before taking it.
Avoid it during pregnancy and breastfeeding. Use only the leaf preparation, never neem oil, especially if children are in the home.

Neem by Herbal Hills Drug Interactions
HelloPharmacist Interaction Report
Neem by Herbal Hills interacts with a broad range of medications through its effects on how your body processes drugs.
The most serious concern is with antidiabetes drugs — neem can lower blood sugar, and when combined with medications that do the same, you risk low blood sugar (hypoglycemia). Altogether, these interactions span 1,014 individual medications.
Read the full breakdown — every affected drug type, severity by severity
Neem leaf extract appears to slow down several liver enzymes (CYP1A2, CYP2C8, CYP2C9, and CYP3A4 substrates) and a kidney transport system (P-glycoprotein substrates) that clear many drugs from your body. This means those medications could build up to higher levels, making their effects stronger — though this reaction hasn't been confirmed in actual patients yet, only in lab studies.
The concern is moderate for all of these. Additionally, neem may weaken immunosuppressants, the drugs given to prevent organ rejection or manage severe autoimmune conditions.
Because neem affects so many drug-clearing pathways, before you start this supplement, check your exact medications with the search tool on this page or discuss it with your own doctor or pharmacist.
