Not established
Neem leaf has possibly effective evidence for gingivitis, dental plaque, and lice. For acne and tungiasis (a parasitic infection), the evidence isn't strong enough to rate.

Well-documented data
Neem leaf preparations are generally well tolerated when taken by mouth in the short term. Most people don't report side effects, though high-quality safety studies haven't been done.
Applied to skin, neem seems well tolerated, though a small number of people with sensitive skin have reported contact dermatitis. Rare serious side effects reported with oral use include cardiac rhythm problems and kidney damage in adults; neem oil in children can cause serious harm, so this product is not for children.
One case report noted hemolytic anemia (breakdown of red blood cells) in an adult who drank large amounts of neem tea daily over several weeks and had a genetic condition (G6PD deficiency) that made him vulnerable. The safety data advises against neem during pregnancy because it may affect fertility and pregnancy.

Neem leaf extract may inhibit enzymes (CYP1A2, CYP3A4, CYP2C9, and CYP2C8) that your liver uses to break down many medications. If neem slows that breakdown, drug levels in your blood could rise and cause stronger effects.
Additionally, neem may interfere with P-glycoprotein transporters, which help move certain drugs out of cells. Altogether, these interactions span 994 individual medications.
Neem can lower blood glucose, so if you take diabetes medications, there's a risk of low blood sugar (hypoglycemia). It may also reduce how well immunosuppressants work, which matters if you take them after a transplant or for an autoimmune condition.

Neem
Antidiabetes Drugs
Neem might increase the risk of hypoglycemia when taken with antidiabetes drugs.
Some clinical research shows that neem can lower blood glucose levels in adults with type 2 diabetes, including those already taking metformin.
Cytochrome P450 1A2 (Cyp1A2) Substrates
Theoretically, neem leaf extract might increase the levels and clinical effects of CYP1A2 substrates.
In vitro research shows that neem leaf extract inhibits CYP1A2 enzymes. So far, this reaction has not been reported in humans.
Cytochrome P450 2C8 (Cyp2C8) Substrates
Theoretically, neem leaf extract might increase the levels and clinical effects of CYP2C8 substrates.
In vitro research shows that neem leaf methanol extract inhibits CYP2C8 enzymes. So far, this reaction has not been reported in humans.
Cytochrome P450 2C9 (Cyp2C9) Substrates
Theoretically, neem leaf extract might increase the levels and clinical effects of CYP2C9 substrates.
In vitro research shows that neem leaf methanol extract inhibits CYP2C9 enzymes. So far, this reaction has not been reported in humans.
Cytochrome P450 3A4 (Cyp3A4) Substrates
Theoretically, neem leaf extract might increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that neem leaf methanol extract inhibits CYP3A4 enzymes. So far, this reaction has not been reported in humans.
Immunosuppressants
Theoretically, neem might decrease the effectiveness of immunosuppressants.
Animal research suggests that neem might have immunostimulant effects.
P-Glycoprotein Substrates
Theoretically, neem leaf extract might increase the levels and clinical effects of P-glycoprotein substrates.
In vitro research shows that neem leaf methanol extract inhibits renal P-glycoprotein transport activity. So far, this reaction has not been reported in humans.
