The neem in this product is possibly effective for gingivitis (gum inflammation), lice, and dental plaque. There's insufficient reliable evidence to know whether it works for acne or tungiasis (a parasitic skin infection).
If you're considering it for gum health or lice, the evidence leans cautiously in its favor — but talk with your doctor or dentist about whether it's right for your situation.
How safe is it?
Well-documented data
Neem leaf preparations are generally well tolerated short-term in adults when taken by mouth, though high-quality safety studies haven't been done. The most common topical side effect is contact dermatitis in people with sensitive skin.
Rarely, serious effects like cardiac arrest, kidney damage, and heart rhythm problems have been reported with oral neem leaf in adults. Neem leaf can also cause hemolysis (breakdown of red blood cells), especially in people with G6PD deficiency.
In children, neem oil is particularly risky and can cause serious harm. Neem should be avoided during pregnancy because it may affect fertility and pregnancy.
There isn't enough safety data for breastfeeding, so it's best avoided then too. If you're pregnant, planning pregnancy, or breastfeeding, talk with your doctor or pharmacist before starting.

The most serious concern is with drugs metabolized by your liver's CYP3A4 enzyme (a major pathway for hundreds of medications) — neem may raise their blood levels and effects. This is still theoretical based on lab work, not yet reported in humans, but worth checking.

Neem also affects five other drug-processing pathways: CYP1A2, CYP2C8, CYP2C9 substrates, and P-glycoprotein substrates (a transporter that moves drugs in and out of cells). All of these are Moderate severity and similarly backed by in vitro data without human reports so far.
Additionally, neem may increase your risk of low blood sugar (hypoglycemia) if you take diabetes medications — some clinical research shows neem can lower blood glucose on its own. It may also theoretically reduce the effectiveness of immunosuppressants used after transplant or for autoimmune conditions.
Altogether, these interactions span 994 individual medications. Because neem's effects happen through enzyme inhibition, the list is broad.

Neem leaves extract
Antidiabetes Drugs
Neem might increase the risk of hypoglycemia when taken with antidiabetes drugs.
Some clinical research shows that neem can lower blood glucose levels in adults with type 2 diabetes, including those already taking metformin.
Cytochrome P450 1A2 (Cyp1A2) Substrates
Theoretically, neem leaf extract might increase the levels and clinical effects of CYP1A2 substrates.
In vitro research shows that neem leaf extract inhibits CYP1A2 enzymes. So far, this reaction has not been reported in humans.
Cytochrome P450 2C8 (Cyp2C8) Substrates
Theoretically, neem leaf extract might increase the levels and clinical effects of CYP2C8 substrates.
In vitro research shows that neem leaf methanol extract inhibits CYP2C8 enzymes. So far, this reaction has not been reported in humans.
Cytochrome P450 2C9 (Cyp2C9) Substrates
Theoretically, neem leaf extract might increase the levels and clinical effects of CYP2C9 substrates.
In vitro research shows that neem leaf methanol extract inhibits CYP2C9 enzymes. So far, this reaction has not been reported in humans.
Cytochrome P450 3A4 (Cyp3A4) Substrates
Theoretically, neem leaf extract might increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that neem leaf methanol extract inhibits CYP3A4 enzymes. So far, this reaction has not been reported in humans.
Immunosuppressants
Theoretically, neem might decrease the effectiveness of immunosuppressants.
Animal research suggests that neem might have immunostimulant effects.
P-Glycoprotein Substrates
Theoretically, neem leaf extract might increase the levels and clinical effects of P-glycoprotein substrates.
In vitro research shows that neem leaf methanol extract inhibits renal P-glycoprotein transport activity. So far, this reaction has not been reported in humans.
