Nattokinase is generally well tolerated in short-term use, but its blood-thinning effect means you should be cautious if you have any bleeding risk. Rare cases of bleeding have been reported, including a documented case of brain hemorrhage when nattokinase was combined with aspirin.

It can also trigger allergic reactions (including severe ones) in people sensitive to natto or its components; reactions typically appear within 2 hours and affect the throat and voice box. There isn't enough safety data to recommend it during pregnancy, so it's best avoided.

Safety while breastfeeding is unknown, so it's advisable to avoid it then as well.
