Phosphatidylserine is rated possibly effective for age-related cognitive decline and Alzheimer disease based on available evidence. For athletic performance, attention deficit-hyperactivity disorder (ADHD), cognitive function in general, and dementia, the evidence is insufficient to establish how well it works.

How safe is it?
Well-documented data
Phosphatidylserine is generally well tolerated when taken by mouth. The most common side effects reported in studies are flatulence, gastrointestinal upset, headache, insomnia, and nausea.
Insomnia is more likely at higher doses (around 600 mg), and gastrointestinal upset can occur at doses of 200–300 mg per day. Long-term safety hasn't been fully established.
The safety data advises against use during pregnancy and breastfeeding, as there isn't enough information to know it's safe in these situations. If you're pregnant, planning to become pregnant, or breastfeeding, talk with your doctor or pharmacist before taking this supplement.
