Does it work?
Moderate evidence
The evidence on what pine bark extract actually does is mixed. It's possibly effective for osteoarthritis (joint pain and stiffness) and for chronic venous insufficiency (poor circulation in the legs causing swelling and discomfort).
For asthma, the data also rates it possibly effective. However, for high cholesterol (hyperlipidemia), the evidence suggests it's possibly ineffective — so don't count on it to lower your lipid numbers.
How safe is it?
Well-documented data
Maritime pine bark extract is generally well tolerated in studies, though long-term safety hasn't been fully worked out. The most common side effects when taken by mouth are stomach upset, dizziness, and vertigo.
Headache has been reported in one trial. A couple of rare cases of chest pain and heart failure worsening have been reported, but both patients had serious heart history already, so it's unclear whether the extract caused them.
There isn't enough safety data to know whether it's safe in pregnancy or while breastfeeding — the facts advise against use in both situations. Talk with your doctor or pharmacist if you're pregnant, planning to become pregnant, or nursing before taking this.

Anticoagulant/Antiplatelet Drugs
Theoretically, maritime pine bark extract might increase the risk of bleeding when used with antiplatelet or anticoagulant drugs.
Clinical research suggests that maritime pine bark extract inhibits platelet aggregation. However, the clinical significance of this effect is unclear.
Antidiabetes Drugs
Theoretically, maritime pine bark extract might increase the risk of hypoglycemia when used with antidiabetes drugs.
One clinical study shows that maritime pine bark extract decreases blood sugar in patients with diabetes being treated with antidiabetes agents. Monitor blood glucose levels closely. Dose adjustments might be necessary.
Immunosuppressants
Theoretically, maritime pine bark extract might decrease the effectiveness of immunosuppressant therapy.
In vitro and animal research suggests that maritime pine bark extract has immunostimulant activity. This effect has not been reported in humans.
