Lactobacillus acidophilus is generally well tolerated in healthy adults and children. Uncommon gastrointestinal side effects — abdominal discomfort, pain, indigestion, gas, bloating, diarrhea, vomiting, and burping — can occur when taking it alongside other probiotics.

Because these are live organisms, there's a theoretical concern they could cause serious infection (bacteremia, sepsis, abscess, or meningitis) in patients who are severely ill or immunocompromised; most documented cases involve translocation from other parts of the body where these bacteria naturally live. Lactobacillus bulgaricus carries similar cautions.

Streptococcus thermophilus is generally well tolerated but poses comparable infection risk in patients with weakened immune systems, central lines, or severe illness. For pregnancy, Lactobacillus acidophilus and Streptococcus thermophilus are rated possibly safe; Lactobacillus bulgaricus has limited pregnancy data.
