The evidence is mixed and often limited. Lactobacillus acidophilus shows possibly effective ratings for irritable bowel syndrome (IBS), Helicobacter pylori infection, antibiotic-associated diarrhea, and bacterial vaginosis.
Bifidobacterium bifidum is possibly effective for IBS and respiratory tract infections. Saccharomyces boulardii is possibly effective for rotaviral diarrhea, travelers' diarrhea, antibiotic-associated diarrhea, necrotizing enterocolitis, Clostridioides difficile infection, and Helicobacter pylori.
For other claimed uses—like allergic rhinitis, asthma, anxiety, cognitive decline, and constipation—the evidence is insufficient to rate them reliably, or the data show these strains are possibly ineffective (as with L. acidophilus and atopic disease).
How safe is it?
Well-documented data
Lactobacillus acidophilus and the Bifidobacterium species are generally well tolerated in healthy adults and children, though caution is advised for people with weakened immune systems or serious illness. Rare gastrointestinal side effects have been reported with these probiotics, including bloating, flatulence, abdominal pain, and diarrhea; these are uncommon.
Very rarely, live probiotic organisms have caused serious infections like bacteremia or sepsis, mostly in critically ill or immunocompromised patients. FOS (the prebiotic) commonly causes abdominal pain, bloating, and flatulence at higher doses, though clinical studies suggest doses under 10 grams daily are mild.
Saccharomyces boulardii is generally safe for healthy adults but should be avoided by people who are critically ill or immunocompromised, because rare fungal infections have occurred. Bifidobacterium longum is possibly safe in pregnancy and lactation; Saccharomyces boulardii's pregnancy and lactation data are on file but not specified.

Theoretically, taking Lactobacillus acidophilus with antibiotic drugs might decrease the effectiveness of L. acidophilus.
L. acidophilus preparations usually contain live and active organisms. Therefore, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and L. acidophilus preparations by at least two hours.
Bifidobacterium longum infantis
Antibiotic Drugs
Theoretically, taking Bifidobacterium longum with antibiotic drugs might decrease the effectiveness of B. longum.
Since B. longum preparations usually contain live and active organisms, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and B. longum preparations by at least 2 hours.
Bifidobacterium bifidum
Antibiotic Drugs
Theoretically, taking Bifidobacterium. bifidum with antibiotic drugs might decrease the effectiveness of B. bifidum.
Since B. bifidum preparations usually contain live and active organisms, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and B. bifidum preparations by at least 2 hours.
Saccharomyces boulardii
Antifungals
Theoretically, taking antifungals with Saccharomyces boulardii might decrease the effectiveness of Saccharomyces boulardii.
S. boulardii is a live yeast. Therefore, simultaneously taking antifungals might kill a significant number of the organisms.
