Evidence for these ingredients is limited. The carnitine forms are effective for treating carnitine deficiency but are only possibly effective for heart failure, angina, and high cholesterol—meaning the research shows promise but isn't conclusive.
Garcinia and Raspberry Ketones show insufficient reliable evidence to rate for obesity and athletic performance, so we don't know whether they work for weight loss at the doses in this product. Acetyl-L-Carnitine is possibly effective for age-related cognitive decline, Alzheimer's disease, diabetic nerve pain, and depression, though again the evidence isn't definitive.
How safe is it?
Well-documented data
The carnitine ingredients are generally well tolerated, though high doses can cause stomach upset, nausea, diarrhea, abdominal pain, and a fishy body odor. A rare serious side effect is seizures.
Acetyl-L-Carnitine may cause agitation, dry mouth, headache, insomnia, and reduced appetite. Garcinia is generally tolerated short-term, but rare reports link it to liver injury, mania, and pancreatitis.
Raspberry Ketones structurally resemble a stimulant and have been linked to heart palpitations, elevated blood pressure, and dangerous heart rhythms in case reports, though human safety data is very limited. Pregnancy: avoid Raspberry Ketones and Garcinia; safety data for L-Carnitine in pregnancy is absent.
Lactation: L-Carnitine is possibly safe; avoid Raspberry Ketones and Garcinia; safety for Acetyl-L-Carnitine and Propionyl-L-Carnitine while breastfeeding is unknown.

L-Carnitine L-Tartrate and its related compounds (Acetyl-L-Carnitine HCl and Propionyl-L-Carnitine HCl) may increase the blood-thinning effects of warfarin (Coumadin) and acenocoumarol (Sintrom). There's also a theoretical concern that these carnitine forms could decrease how well thyroid hormone replacement works by blocking the hormone from entering cells properly.
Raspberry Ketones may amplify the effects of stimulant drugs (including some decongestants and ADHD medications), raising the risk of heart palpitations, elevated blood pressure, and dangerous heart rhythm changes. Raspberry Ketones may also increase warfarin dose requirements, as documented in a case report where a patient needed a significantly higher warfarin dose while taking raspberry ketone.
Garcinia cambogia extract may increase serotonergic side effects (including serotonin syndrome) when combined with antidepressants and other serotonergic drugs. It may also increase bleeding risk with blood thinners (anticoagulants) or antiplatelet drugs, raise the risk of low blood sugar (hypoglycemia) with diabetes medications, and theoretically increase liver damage risk when taken with other hepatotoxic drugs.
Acetyl-L-Carnitine HCl carries similar blood thinner and thyroid hormone interactions as the other carnitine forms, plus the serotonergic drug concern noted for Garcinia. We could not check Clarinol.

Garcinia cambogia extract
Anticoagulant/Antiplatelet Drugs
Theoretically, hydroxycitric acid (HCA), the main active ingredient in garcinia, might increase the risk of bleeding when used with antiplatelet or anticoagulant drugs.
HCA inhibits platelet aggregation in vitro. The inhibitory effect seems to be greater in platelets extracted from diabetic subjects than non-diabetic subjects.
Antidiabetes Drugs
Theoretically, hydroxycitric acid (HCA), the main active ingredient in garcinia, might have additive effects with antidiabetes drugs and increase the risk of hypoglycemia.
HCA reduces fasting and postprandial blood glucose levels in animal models, theoretically by delaying glucose absorption. This effect has not been reported in humans.
Hepatotoxic Drugs
Theoretically, concomitant use with other potentially hepatotoxic drugs might increase the risk of developing liver damage.
There have been reports of acute hepatitis with elevated liver enzymes associated with garcinia, when taken alone or in combination with other ingredients. Case reports collected from the Drug Induced Liver Injury Network suggest this risk may be greater in people who carry the HLA B*35:01 allele.
Serotonergic Drugs
Theoretically, combining garcinia with other serotonergic drugs might increase the risk of serotonergic side effects, including serotonin syndrome.
In one report, a patient experienced serotonin syndrome after taking garcinia extract (60% hydroxycitric acid) 1000 mg daily in combination with escitalopram 20 mg, which had been taken for a year. The patient was switched to sertraline 50 mg daily and again experienced serotonin syndrome.
Acetyl-L-Carnitine HCl
Acenocoumarol (Sintrom)
Theoretically, acetyl-L-carnitine might increase the anticoagulant effects of acenocoumarol.
L-carnitine, the parent compound of acetyl-L-carnitine, might enhance the anticoagulant effects of acenocoumarol, an oral anticoagulant that is similar to warfarin, but shorter-acting. There are at least two case reports of INR elevation when L-carnitine was taken with acenocoumarol. In one case, a 33-year-old male with a previously stable INR had an elevated INR of 4.65 after L-carnitine was started and continued for 10 weeks. INR normalized after discontinuation of the L-carnitine-containing product. It is unclear if such an interaction would also occur with acetyl-L-carnitine.
Serotonergic Drugs
Theoretically, acetyl-L-carnitine might increase the risk of serotonergic side effects, including serotonin syndrome and cerebral vasoconstrictive disorders, when taken with serotonergic drugs.
Animal research shows that acetyl-L-carnitine can increase levels of serotonin in the brain.
Thyroid Hormone
Theoretically, acetyl-L-carnitine might decrease the effectiveness of thyroid hormone replacement.
L-carnitine appears to act as a peripheral thyroid hormone antagonist by inhibiting entry of thyroid hormone into the nucleus of cells. Taking L-carnitine also seems to diminish some of the symptoms of hyperthyroidism. It is unclear if such an interaction would occur with acetyl-L-carnitine.
Warfarin (Coumadin)
Theoretically, acetyl-L-carnitine might increase the anticoagulant effects of warfarin.

Raspberry Ketones
Stimulant Drugs
Theoretically, raspberry ketone might increase the risk of adverse cardiovascular effects with stimulant drugs.
Structurally, raspberry ketone resembles synephrine, a known stimulant agent. Heart palpitations, elevated blood pressure, coronary vasospasm, pulseless electrical activity arrest, and resistant polymorphic ventricular tachycardia have been reported in patients taking raspberry ketone.
Warfarin (Coumadin)
Theoretically, raspberry ketone might increase warfarin dose requirements.
In one case report, a patient taking warfarin 55 mg per week had a decrease in INR over a period of one month while taking raspberry ketone 250 mg daily. A warfarin dose increase to 70 mg per week was necessary to maintain a therapeutic INR while taking raspberry ketone. The mechanism for this potential interaction is not known.
L-Carnitine L-Tartrate
Acenocoumarol (Sintrom)
