Every active ingredient lists its own amount on the label.

Why this rating?
  • The label discloses an exact amount for 1 of its 1 active ingredient.
  • No proprietary blends here — you can verify the dose of every single component.

Herbal Terra's Sang Shen is a liquid extract made from certified organic white mulberry (Morus alba) dried fruit. It contains one active ingredient — the mulberry fruit itself in concentrated form.

The only other ingredient listed is menstruum, which is the liquid used to extract the herb.

Evidence for Intended Use · database check
By FDA rules, dietary supplements can’t claim to treat, cure, or prevent disease — so labels speak in careful marketing language. We discern each product’s intended use from its name, label claims, and label statements, then grade the clinical evidence for that use. How these ratings are computed
Not assessable

This is a single-ingredient White Mulberry product and its label doesn't state a use — see White Mulberry's own graded uses on its monograph.

Why this rating?
  • We looked at the product name, claims, and label statements and couldn't find a stated purpose to grade.

The evidence we hold for white mulberry fruit extract is insufficient to rate it for any of the conditions it's marketed for — hair loss, bone health, common cold, blood sugar control, or cholesterol. That doesn't mean it doesn't work; it means the clinical trial data isn't strong enough yet to say either way.

If you're considering it for one of these reasons, talk with your doctor or pharmacist about whether the evidence available outside our data supports a trial.

How safe is it?

Well-documented data
Safety Information · database check
Well characterized

Adverse-effect, pregnancy, and general safety data are on file for most of these ingredients.

Why this rating?
  • We hold adverse-effect (side-effect) data for 1 of the 1 matched ingredient.
  • Pregnancy & breastfeeding safety ratings cover 1 of 1.
  • General safety write-ups exist for 1 of 1.
  • Remember: this measures how much safety information exists. Thin data is not the same as being safe.

White mulberry fruit is generally well tolerated when eaten as food. In supplement form at higher doses, the safety data is more limited, and there's a real concern about lowering blood sugar too much.

Some people in clinical trials reported bloating, gas, loose stools, or constipation, though these side effects often improved over time. During pregnancy and breastfeeding, there isn't enough safety data — the evidence advises against medicinal amounts, so talk with your doctor or pharmacist before using it if you're pregnant or nursing.

The most serious documented interaction for these ingredients is Moderate. Check your medications for a personalized result.

Why this rating?
  • 1 of the 1 matched ingredient can interact with medications — White Mulberry.
  • The most serious interaction on file is rated Moderate.
  • Some involve high-stakes drug classes: diabetes medications.
  • For scale: 87 individual medications appear in the full list. A big number alone doesn't make a product dangerous — what matters is whether YOUR medication is on it, so run yours through the interaction checker on this page.

Before taking Sang Shen, check with your doctor or pharmacist about any diabetes medications you're on — white mulberry can lower blood sugar and create a moderate risk of going too low. You'll also want to confirm that you don't take any medications that are OCT2 substrates (kidney transporters), particularly metformin; white mulberry may slow their clearance and raise their side effects.

The bottom line

Scorecard at a glanceFully disclosed formula with no assessable stated purpose. Moderate medication interactions have been identified, and safety information is well characterized.

Sang Shen is worth considering if you're interested in white mulberry for its traditional uses, but it does carry a moderate risk with diabetes and certain kidney-clearing medications. If you take any prescription drugs — especially for blood sugar — clear this with your doctor or pharmacist first.

And watch for digestive upset when you start; it usually settles down.

Antidiabetes Drugs

Theoretically, white mulberry leaf might increase the risk of hypoglycemia when taken with antidiabetes drugs.
Clinical research shows that white mulberry leaf can lower blood glucose levels. Advise patients to monitor glucose levels closely.

Organic Cation Transporter 2 (Oct2) Substrates

Theoretically, white mulberry leaf might slow the elimination and increase the adverse effects of drugs which are OCT2 substrates.
Animal research shows that coadministration of white mulberry leaf extract with metformin, an OCT2 substrate, slows the renal elimination of metformin via inhibition of OCT2 activity. OCT2 is expressed in the kidneys and is responsible for transporting cationic drugs into tubular epithelial cells in order to be excreted in the urine.

Sang Shen by Herbal Terra: Common Questions

Does Sang Shen by Herbal Terra interact with any medications?
Yes. Based on its ingredients, Sang Shen has a known interaction with 87 medications. Use the checker to see how it interacts with a specific drug.
How can one product interact with so many drugs?
Sang Shen contains a single active ingredient, and that one ingredient can interact with many different medications on its own. We check it against each medication and show the mechanism responsible.
Where does this information come from?
The product label data comes from the NIH Dietary Supplement Label Database (DSLD); the interaction data is built on the Natural Medicines database and reviewed by HelloPharmacist pharmacists.
Will this help with my blood sugar?
The evidence we hold isn't strong enough to rate it for diabetes. White mulberry does lower blood sugar in research, which is exactly why it's risky to combine with diabetes drugs — but whether it helps control blood sugar as a standalone treatment isn't established in our data. Talk it over with your doctor.
Can I take this if I'm pregnant or breastfeeding?
The safety data advises against medicinal amounts during pregnancy and while breastfeeding. There isn't enough information to know if it's safe at these doses. Check with your doctor or pharmacist to discuss your specific situation.
What side effects should I watch for?
The most common ones are bloating, gas, loose stools, and constipation — but they're often temporary. In one clinical study, about half of people reported bloating or gas, a quarter had loose stools, and about one in five had constipation, though these side effects decreased over the course of the study.
Is this safe to take long term?
White mulberry fruit is generally well tolerated when used as food or supplement, but concentrated supplements are less studied than the whole food. If you're thinking about taking it long term, especially if you take any medications, run it by your doctor or pharmacist first.
What is Sang Shen?
Sang Shen is the traditional name for white mulberry fruit. This product is a liquid extract made from the dried fruit of the Morus alba plant.
