Saw palmetto is generally well tolerated, and side effects are usually mild and reversible. The most common ones reported are abdominal pain, constipation, decreased libido, diarrhea, dizziness, fatigue, headache, nausea, stuffy nose, and vomiting.

Rarely, skin reactions including fixed drug eruptions (localized blisters and erosions) and worsening acne have been reported. A few uncommon cases of heart rhythm changes and blood pressure shifts have also been described.

Avoid saw palmetto during pregnancy — the safety data advises against it because of possible hormonal effects and insufficient safety information. Do not use it while breastfeeding due to insufficient safety data.
