Saw palmetto is generally well tolerated, and side effects when they occur tend to be mild and reversible. The most common ones are abdominal pain, constipation, diarrhea, dizziness, headache, nausea, and decreased libido.

Rarely, cardiovascular effects like high blood pressure, rapid heartbeat, or irregular heartbeat have been reported, and very rarely, serious events like heart attack or severe heart rhythm changes. One case of a skin reaction (blistering and erosions) appeared a few days after starting the supplement and resolved when it was stopped.

Saw palmetto is rated likely unsafe in pregnancy due to possible hormonal effects and lack of safety data, and it's not recommended while breastfeeding because there isn't enough information to know it's safe.
