The most common side effects from oral use are abdominal discomfort, bloating, diarrhea, nausea, and vomiting. Raw shiitake can cause shiitake dermatitis, a distinctive skin eruption with whiplash-like marks that may come with whole-body symptoms.
People who handle shiitake mushrooms extensively can develop allergic contact dermatitis, asthma, or other allergic symptoms. In rare cases, inhalation of shiitake spores has caused a serious lung reaction (hypersensitivity pneumonitis) with difficulty breathing, chest pain, dry cough, and rhinitis.

Theoretically, shiitake mushroom might decrease levels of drugs metabolized by CYP2D6.
In vitro studies suggest that the shiitake mushroom extract AHCC might induce the CYP2D6 enzyme. This effect has not been reported in humans.

Theoretically, taking shiitake mushroom might decrease the effects of immunosuppressive therapy.
