Every active ingredient lists its own amount on the label.

Why this rating?
  • The label discloses an exact amount for 4 of its 4 active ingredients.
  • No proprietary blends here — you can verify the dose of every single component.

This product contains four active ingredients. Thiamine (vitamin B1) is an essential nutrient your body uses for energy and nerve function.

Benfotiamine is a fat-soluble form of thiamine that may be absorbed more readily than standard thiamine. Carnosine is a naturally occurring compound made from two amino acids.

We could not check the fourth ingredient, luteolin, because we hold no data for it. The capsule itself is made from gelatin, microcrystalline cellulose (a plant-based binder), magnesium stearate, and silica — these are inactive ingredients that help hold the product together.

Does it work?

Not established
Evidence for Intended Use · database check
By FDA rules, dietary supplements can’t claim to treat, cure, or prevent disease — so labels speak in careful marketing language. We discern each product’s intended use from its name, label claims, and label statements, then grade the clinical evidence for that use. How these ratings are computed
Not established

The graded evidence we hold for these ingredients covers different conditions than the ones this product is marketed for, so there's no established rating for its stated use.

Why this rating?
  • The label markets this product for: Protects muscle cell membranes.
  • We looked for evidence on: Athletic performance, Muscle recovery, Muscle strength, Muscle function.
  • The closest evidence on file: Carnosine is rated "Insufficient Reliable Evidence To Rate" for Athletic performance (Natural Medicines).

Thiamine is effective for preventing and treating thiamine deficiency and Wernicke-Korsakoff syndrome, a serious brain condition. It's possibly effective for period pain (dysmenorrhea), though the evidence is mixed.

For other uses listed — heart failure and mosquito repellent — the evidence does not support it. Benfotiamine is possibly effective for diabetic nerve pain (peripheral neuropathy caused by diabetes).

For other conditions tested — diabetic kidney damage, alcohol-related disorders, Alzheimer disease, and general peripheral nerve pain — the evidence is either not strong enough to rate or suggests it doesn't help. For carnosine and luteolin, the evidence in our data is not established or insufficient to say whether they work for their proposed uses.

How safe is it?

Well-documented data
Safety Information · database check
Well characterized

Adverse-effect, pregnancy, and general safety data are on file for most of these ingredients.

Why this rating?
  • We hold adverse-effect (side-effect) data for 3 of the 3 matched ingredients.
  • Pregnancy & breastfeeding safety ratings cover 3 of 3.
  • General safety write-ups exist for 3 of 3.
  • Remember: this measures how much safety information exists. Thin data is not the same as being safe.

Thiamine is generally very safe when taken by mouth in normal doses; allergic skin reactions and other hypersensitivity problems are very rare. Thiamine is needed during pregnancy and breastfeeding, but use only supplement doses advised by your doctor or pharmacist.

Benfotiamine was generally well tolerated in studies, though long-term safety isn't fully known — a small number of trial participants reported allergic skin reactions or mild stomach discomfort, but these rates matched placebo. Safety while breastfeeding hasn't been well studied.

Carnosine is generally well tolerated in short-term studies, but long-term safety data are limited; some people have reported decreased appetite, dry mouth, weight changes, foot or joint pain, and rarely pneumonia. The safety data advise against carnosine in pregnancy and breastfeeding due to insufficient information.

Moderate interaction found
Known Interaction Concern · database check
Moderate identified

The most serious documented interaction for these ingredients is Moderate. Check your medications for a personalized result.

Why this rating?
  • 2 of the 3 matched ingredients can interact with medications — Thiamine, Carnosine.
  • The most serious interaction on file is rated Moderate.
  • Some involve high-stakes drug classes: diabetes medications.
  • For scale: 89 individual medications appear in the full list. A big number alone doesn't make a product dangerous — what matters is whether YOUR medication is on it, so run yours through the interaction checker on this page.

Check your medications if you take diabetes drugs (antidiabetes medications) — carnosine may increase the risk of low blood sugar. If you're on trimethoprim (an antibiotic), let your pharmacist know before adding this product.

For luteolin, we hold no interaction data, so we cannot flag any medication concerns for that ingredient.

The bottom line

Scorecard at a glanceFully disclosed formula with no established evidence rating for its marketed use. Moderate medication interactions have been identified, and safety information is well characterized.

This product is most relevant if you're looking to support thiamine status or have diabetic nerve pain — though for the latter, benfotiamine's evidence is only modest. If you take diabetes medications or the antibiotic trimethoprim, or if you're pregnant or breastfeeding, talk with your pharmacist before starting.

The long-term safety of several ingredients here isn't well established, so it's worth a conversation.

Trimethoprim (Proloprim)

Trimethoprim might increase blood levels of thiamine.
In vitro, animal, and clinical research suggest that trimethoprim inhibits intestinal thiamine transporter ThTR-2, hepatic transporter OCT1, and renal transporters OCT2, MATE1, and MATE2, resulting in paradoxically increased thiamine plasma concentrations.

Super Carnosine 500 mg by Life Extension: Common Questions

Does Super Carnosine 500 mg by Life Extension interact with any medications?
Yes. Based on its ingredients, Super Carnosine 500 mg has a known interaction with 89 medications. Use the checker to see how it interacts with a specific drug.
How can one product interact with so many drugs?
Super Carnosine 500 mg contains 4 active ingredients, and an interaction can come from any of them. We check every ingredient, combine the results into one list per medication, and show which ingredient and mechanism is responsible.
Where does this information come from?
The product label data comes from the NIH Dietary Supplement Label Database (DSLD); the interaction data is built on the Natural Medicines database and reviewed by HelloPharmacist pharmacists.
Can I take this if I'm pregnant or breastfeeding?
Thiamine is needed during pregnancy and breastfeeding and can be used at supplement doses under your doctor's guidance. Benfotiamine doesn't have enough safety data, so avoid it unless your doctor says otherwise. Carnosine — we don't have enough safety information — the data advise against use in pregnancy and while breastfeeding. Talk with your pharmacist or OB about whether this product is right for you.
Does this actually work for brain health or aging?
The evidence in our data for these uses is insufficient or not established. Thiamine is proven for deficiency and a specific brain disorder (Wernicke-Korsakoff syndrome), and possibly helpful for period pain. The other ingredients haven't shown reliable evidence for cognitive decline, Alzheimer disease, or general aging in the studies we have.
What are the most common side effects?
All three ingredients with safety data were generally well tolerated. With carnosine, some people reported decreased appetite, dry mouth, weight changes, or joint pain. Benfotiamine caused mild stomach discomfort in a few trial participants. Thiamine allergic reactions are very rare.
Why is thiamine in here if the product is called Super Carnosine?
Thiamine (vitamin B1) supports energy and nerve function, so it may complement carnosine's intended role. Benfotiamine, a special form of thiamine, is also included — it's absorbed differently than regular thiamine and is studied for diabetic nerve pain.
Is this safe if I take diabetes medication?
Carnosine in this product may increase the risk of low blood sugar (hypoglycemia) when combined with diabetes drugs. Don't start it without talking to your pharmacist or doctor first — they can assess your individual risk and monitor you if you do take it together.
What is luteolin, and does it interact with anything?
Luteolin is a plant-based compound in this product. We don't hold interaction data for it, so we can't tell you whether it affects any medications. Use the medication checker on this page to search your drugs, or ask your pharmacist to look it up.
