Our pharmacy team’s full take, with four database checks built into the cards below — a summary of what is known, not a grade of the product itself.
What’s inside
Full disclosure
KAL Sure Stevia Extract is a single-ingredient product: stevia leaf extract, a natural sweetener derived from the stevia plant. This ingredient has a long history as a food sweetener.
The product contains no inactive fillers or binders — it's pure stevia extract powder.
Does it work?
Couldn't assess
The evidence for stevia's effectiveness in managing diabetes, high blood pressure, or weight is insufficient. While some preliminary research suggests stevia extract might lower blood sugar after meals and affect blood pressure, other clinical studies show no significant effect.
We don't hold enough reliable data to rate whether it actually works for these purposes.
How safe is it?
Well-documented data
Purified stevia in food amounts is generally well tolerated and recognized as safe. However, this is a supplement-dose powder, not a food amount, and high-dose supplements haven't been well studied — so use caution.
The most common minor side effects are abdominal bloating, dizziness, headache, muscle pain, nausea, and numbness; most resolve within the first week. Serious allergic reactions are rare.
There is a caution with whole-leaf stevia and high-dose supplements due to limited safety data. Pregnancy and lactation safety information is not on file for this ingredient.
Meds to double-check
Moderate interaction found
If you take lithium, any blood pressure medication, or any diabetes drug, double-check with your doctor or pharmacist before using this product. Stevia may raise lithium levels and could lower blood pressure or blood sugar further.
The bottom line
Scorecard at a glanceFully disclosed formula with no assessable stated purpose. Moderate medication interactions have been identified, and safety information is well characterized.
KAL Sure Stevia Extract is a natural sweetener, but it's not proven to lower blood sugar, blood pressure, or weight. If you take lithium, blood pressure medication, or diabetes drugs, talk with your doctor or pharmacist before starting — the product may affect how these work.
For healthy people with no medications, it's generally well tolerated, though long-term high-dose safety isn't established.

Sure Stevia Extract by KAL Drug Interactions
HelloPharmacist Interaction Report
KAL Sure Stevia Extract contains stevia leaf extract, which interacts with lithium, blood pressure medications, and diabetes drugs.
The most serious concern is lithium: stevia may slow how your body clears it, potentially raising lithium levels to unsafe amounts. This is a Moderate-severity interaction.
Read the full breakdown — every affected drug type, severity by severity
Stevia may also lower blood pressure further when combined with antihypertension medications (Minor severity), and could theoretically increase the risk of low blood sugar (hypoglycemia) if you take diabetes drugs (Minor severity). The research is mixed — some studies show stevia affects blood sugar and blood pressure, others don't — but the theoretical risk is there.
Before you start this product, check your exact medications with the search tool on this page, especially if you take lithium, any blood pressure medication, or diabetes medication.

For each ingredient in Sure Stevia Extract with known interactions, here are the types of medications it can affect. Open any type for the detail — or search your exact drug in the checker above.
Stevia Leaf Extract
Lithium
Theoretically, stevia might decrease clearance and increase levels of lithium.
Animal research suggests that stevia extracts might have diuretic activity. Theoretically, increased reabsorption of lithium along with sodium might reduce excretion and increase levels of lithium.
Antidiabetes Drugs
Theoretically, stevia might increase the risk for hypoglycemia when combined with antidiabetes drugs.
Preliminary clinical research in patients with type 2 diabetes suggests that taking a single dose of stevia extract 1000 mg reduces postprandial blood glucose levels when taken with a meal. However, other clinical research in patients with type 1 or type 2 diabetes suggests that taking stevioside 250 mg three times daily does not significantly affect blood glucose levels or glycated hemoglobin (HbA1C) after three months of treatment.
Antihypertensive Drugs
Theoretically, combining stevia or stevia constituents with antihypertensive agents might increase the risk of hypotension.
Stevia extract and stevioside might lower blood pressure in patients with hypertension. However, other clinical research suggests that stevioside does not significantly lower blood pressure in patients with hypertension.
