Strong evidence
The evidence for most of these ingredients is limited. Pygeum is likely effective for benign prostatic hyperplasia (enlarged prostate), but saw palmetto is actually marked possibly ineffective for BPH despite common use for it.
Holy Basil, Stinging Nettle, Magnolia, and Piper cubeba don't have established evidence for the conditions this product seems aimed at — the data we hold shows insufficient reliable evidence to rate them for anxiety, diabetes, or prostate health. How safe is it?
Well-documented data
Holy Basil, Saw Palmetto, Stinging Nettle, and Magnolia are all generally well tolerated short-term in adults, though long-term safety data are limited. The most common side effects reported with these ingredients are mild — loose stools and nausea with Holy Basil; abdominal pain, constipation, diarrhea, headache, and nausea with Saw Palmetto; constipation or diarrhea with Stinging Nettle.
Magnolia has been well tolerated in trials, though one patient in a combination study reported sexual dysfunction. During pregnancy, Holy Basil and Saw Palmetto are marked likely or possibly unsafe — animal data raise concerns and safety isn't established, so they should be avoided.
Stinging Nettle and Magnolia haven't been adequately studied in pregnancy, and traditional concerns about their effects on the uterus mean they're best avoided too. None of these ingredients have enough safety data during breastfeeding — your doctor or pharmacist can help you weigh whether this product is right for you if you're nursing.

Birth control or estrogen therapy may be less effective with Saw Palmetto. Warfarin users should be aware that Stinging Nettle's vitamin K content could interfere with it.

Suggested Usage: As a dietary supplement, adults may take 2 capsules each day with food for 1 to 2 weeks or as directed by your healthcare professional. The dose may then be increased to 4 capsules each day with food for 2 to 4 months or as directed by your healthcare professional. After 2 to 4 months, dosage may be lowered back down to 2 capsules each day with food and may continue on that dosage as needed or as directed by your healthcare professional.

Holy Basil and Magnolia bark extract both interact with blood thinners (anticoagulant or antiplatelet drugs) — they may increase your bleeding risk. Holy Basil also affects blood sugar control with antidiabetes drugs, raising the risk of low blood sugar (hypoglycemia), and theoretically increases the sedative effects of pentobarbital.
Saw Palmetto berry extract can reduce how well estrogens and birth control pills work, and also increases bleeding risk with blood thinners.
Stinging Nettle leaf extract has additive effects with antidiabetes drugs (low blood sugar risk), water pills (diuretics), and lithium — and because it contains vitamin K, it may interfere with warfarin. Magnolia bark extract interacts with sedatives and CNS depressants (central nervous system depressants), potentially adding to their drowsiness effect.

Piper cubeba
Cytochrome P450 3A4 (Cyp3A4) Substrates
Several compounds isolated from cubebs can inhibit cytochrome P450 3A4 (CYP3A4) in vitro. So far, this effect has not been reported in humans. Theoretically, cubebs might increase the effects and side effects of drugs metabolized by CYP3A4. These include lovastatin (Mevacor), clarithromycin (Biaxin), cyclosporine (Neoral, Sandimmune), diltiazem (Cardizem), estrogens, indinavir (Crixivan), triazolam (Halcion), and numerous others.
Antacids
Theoretically, due to reports that cubebs increases stomach acid, cubebs might decrease the effectiveness of antacids.
H2-Blockers
Theoretically, due to reports that cubebs increases stomach acid, cubebs might decrease the effectiveness of H2-blockers. The H2 blockers include cimetidine (Tagamet), ranitidine (Zantac), nizatidine (Axid), and famotidine (Pepcid).
Proton Pump Inhibitors (Ppis)
Theoretically, due to reports that cubebs increases stomach acid, cubebs might decrease the effectiveness of PPIs. PPIs include omeprazole (Prilosec), lansoprazole (Prevacid), rabeprazole (Aciphex), pantoprazole (Protonix), and esomeprazole (Nexium).
Magnolia officinalis bark extract
Anticoagulant/Antiplatelet Drugs
Theoretically, magnolia might have additive effects and increase the risk of bleeding when used with anticoagulant or antiplatelet drugs.
In vitro research shows that the chemicals magnolol and honokiol, isolated from magnolia bark, inhibit platelet aggregation that is experimentally induced by collagen and arachidonic acid. However, they do not inhibit platelet aggregation that is induced by adenosine diphosphate, platelet-activating factor, or thrombin. This interaction has not been reported in humans.
Cns Depressants
Theoretically, concomitant use of large doses of magnolia bark and CNS depressants might have additive effects.
In vitro and animal research shows that constituents extracted from magnolia bark, especially honokiol and magnolol, have sedative effects. These effects may be due to the inhibition of catecholamine release and modulation of gamma-aminobutyric acid-A (GABA-A) receptors.
Holy Basil
Anticoagulant/Antiplatelet Drugs
Theoretically, holy basil seed oil might increase the risk of bleeding when used with anticoagulant or antiplatelet drugs.
Animal research shows that holy basil seed oil can prolong bleeding time, possibly due to inhibition of platelet aggregation. However, it is not known if this occurs in humans.
Antidiabetes Drugs
Theoretically, holy basil might increase the risk of hypoglycemia when taken with antidiabetes drugs.
Small clinical studies show that taking holy basil can decrease fasting blood glucose and other measures of glycemic control in patients with type 2 diabetes.
Pentobarbital (Nembutal)
Theoretically, holy basil seed oil might increase the sedative effects of pentobarbital.
Animal research shows that holy basil seed oil increases pentobarbitone-induced sleeping time. However, it is not known if this occurs in humans or if this applies to other barbiturates or sedatives.
Saw Palmetto berry extract
Anticoagulant/Antiplatelet Drugs
Saw palmetto might increase the risk of bleeding with anticoagulant or antiplatelet drugs.
Saw palmetto is reported to prolong bleeding time. Theoretically, it might increase the risk of bleeding when used concomitantly with anticoagulant or antiplatelet drugs.
Contraceptive Drugs
Saw palmetto might reduce the effectiveness of contraceptive drugs.
Saw palmetto might have antiestrogenic effects. Theoretically, it might interfere with contraceptive drugs taken concomitantly.
Estrogens
Saw palmetto might reduce the effectiveness of estrogens.
Saw palmetto might have antiestrogenic effects. Theoretically, it might interfere with estrogens taken concomitantly.
Stinging Nettle leaf extract
Antidiabetes Drugs
Theoretically, stinging nettle might have additive effects with antidiabetes drugs.
Clinical research shows that stinging nettle might decrease blood glucose levels in patients with diabetes.
Diuretic Drugs
Theoretically, combining stinging nettle with diuretic drugs may have additive effects.
Animal research suggests that the above ground parts and roots of stinging nettle may have a diuretic effect.
Lithium
Theoretically, stinging nettle might reduce excretion and increase levels of lithium.
Animal research suggests that stinging nettle has diuretic and natriuretic properties, which could alter the excretion of lithium. The dose of lithium might need to be decreased.
Warfarin (Coumadin)
There is some concern that stinging nettle might decrease the effects of anticoagulant drugs such as warfarin.
Stinging nettle contains a significant amount of vitamin K. When taken in large quantities, this might interfere with the activity of warfarin.
