How safe is it?
Well-documented data
Lemon balm is generally well tolerated for short-term use, though dizziness and sedation are reported; however, these occurred no more often than placebo in clinical trials. Long-term safety is not well studied.
Bugleweed may affect thyroid function and should only be used under healthcare provider guidance. Motherwort is generally tolerated orally in normal amounts, though doses above 3 grams have caused uterine bleeding; common oral side effects are diarrhea and stomach irritation.
Cactus Grandiflorus can cause mouth burning, nausea, vomiting, and diarrhea when taken orally. Pregnancy safety is a concern across all four ingredients — lemon balm has insufficient data; bugleweed and motherwort are rated likely unsafe; and Cactus Grandiflorus should be avoided.
Bugleweed may reduce milk production and should not be used while breastfeeding; motherwort and Cactus Grandiflorus also lack reliable breastfeeding safety data.

Lemon Balm aerial parts extract
Cns Depressants
Theoretically, concomitant use of lemon balm might have additive effects with CNS depressant drugs.
Lemon balm seems to have CNS depressant activity in animals and in humans.
Thyroid Hormone
Theoretically, lemon balm might interfere with thyroid hormone replacement therapy.
In vitro, constituents of lemon balm extract bind to thyroid stimulating hormone (TSH), preventing TSH receptor-binding and leading to the inhibition of TSH-stimulated adenylate cyclase activity. In animals, lemon balm extract has been shown to decrease levels of circulating TSH and inhibit thyroid secretion.
Motherwort Flowering Aerial Parts Extract
Cns Depressants
Theoretically, taking motherwort concomitantly with other CNS depressants may increase the risk of sedation.
Motherwort has been reported to cause CNS depression and sedation.
Bugleweed flowering aerial parts extract
Thyroid Hormone
Theoretically, bugleweed may alter thyroid function.
Concomitant use of bugleweed may reduce the effects of thyroid hormones by blocking peripheral conversion of thyroxine (T4) to triiodothyronine (T3).
Cactus Grandiflorus extract
Digoxin (Lanoxin)
Cereus may potentiate the actions of cardiac glycosides and may enhance the effect of other cardiac drugs.
Monoamine Oxidase Inhibitors (Maois)
Theoretically, excessive doses of cereus may interact with MAOIs, because of the tyramine content.
