Tribulus is generally well tolerated short-term in healthy adults, but long-term safety data and quality standards are limited. Serious side effects are rare but have been reported in individual cases, including liver and kidney injury, seizures, and in one extreme case (consuming 2 liters of tribulus water daily for two days), acute kidney injury and hepatitis.

More common—though still uncommon—are mild gastrointestinal complaints like stomach pain, nausea, diarrhea, or constipation; however, in clinical trials these rates matched placebo. Do not use this product during pregnancy because it may affect hormones and safety is not established.

Avoid it while breastfeeding due to lack of reliable safety information.
