Lactobacillus acidophilus is generally well tolerated in healthy adults and children. Uncommon gastrointestinal side effects can occur when taking it with other probiotics — including stomach discomfort, pain, bloating, flatulence, diarrhea, vomiting, and burping.

Because these are live microorganisms, there's a theoretical concern they could cause serious infections in certain patients, though most documented cases involved bacteria that translocated from other parts of the body where they naturally live. A small number of trial participants taking a combination probiotic had rash or itching, though it's unclear whether these were caused by the L. acidophilus itself or other factors.
