Wild cherry is generally considered safe in the small amounts found in commercial cough products, but the plant contains compounds that can form cyanide in your body, and the safety data advises caution with larger doses. In large amounts—particularly concentrated preparations—wild cherry can cause cyanide toxicity, which can be serious or fatal.
A documented case involved a woman who consumed about 300 grams of wild cherries steeped in alcohol and developed severe symptoms including headache, nausea, vomiting, confusion, and difficulty breathing. Safety hasn't been established during breastfeeding, so the safety data advises against use at that time.

Cytochrome P450 3A4 (Cyp3A4) Substrates
In vitro research suggests that wild cherry can inhibit cytochrome P450 3A4 (CYP3A4) enzymes. Theoretically, wild cherry might increase levels of drugs metabolized by CYP3A4. However, so far, this interaction has not been reported in humans.
Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), ketoconazole (Nizoral), itraconazole (Sporanox), fexofenadine (Allegra), triazolam (Halcion), and others.
