How we grade severity & evidence
Severity levels
- Contraindicated: These should generally not be used together.
- Major: Potentially serious — often needs a change or close monitoring.
- Moderate: Can be significant — usually manageable with monitoring.
- Minor: Usually limited clinical impact.
Evidence grades
- Established: Well documented — supported by controlled studies or strong clinical data.
- Probable: Good supporting evidence, though not definitively proven.
- Suspected: Some evidence suggests this interaction, but it is not well established.
- Possible: Limited or conflicting evidence; the interaction may occur.
- Theoretical: Predicted from the drugs' pharmacology; not yet confirmed in people.
Ratings come from the documented interaction literature and are reviewed by a pharmacist. They describe the documented risk of the combination, not what will necessarily happen to you — your dose, timing, and health picture all matter.
What happens
Reduced bosutinib exposure
Interaction Deep Dive
Avoid concomitant use of bosutinib with drugs that elevate gastric pH, such as proton pump inhibitors, because it may reduce the plasma concentrations of bosutinib. Bosutinib is soluble at a pH of 5 or less, and solubility is greatly decreased with pH levels greater than 5. During a drug interaction study, coadministration of bosutinib and lansoprazole resulted in a decrease in bosutinib AUC and Cmax. Whenever possible, consider using short-acting antacids or H2 blockers and separate dosing with bosutinib by at least 2 hours1.
Why it happens (mechanism)
Interference with bosutinib absorption due to increased gastric pH caused by proton pump inhibitors

a) During a cross-over drug interaction study in healthy volunteers (n=23), coadministration of single dose bosutinib 400 mg and multiple dose lansoprazole 60 mg under fasted conditions resulted in a 26% and 46% decrease in bosutinib AUC and Cmax 1.
