Acai and Tofacitinib

We checked these two against our interaction database. Here is what we found.

No Known Interaction Found

We did not find a documented interaction between Tofacitinib and Acai in our sources. This does not guarantee that the combination is completely free of risk — there is often limited research on supplements and herbal products, especially around drug interactions.

If you decide to take both, the practical routine is simple: add one new thing at a time and note the date you started it. If the combination is going to disagree with you, it usually shows in the first days to a couple of weeks — and a start date turns “I’ve been feeling off” into something a pharmacist can actually work with. Do not stop a prescribed medicine on your own.

Always talk with your pharmacist or doctor before making changes to your medications or supplements, particularly if you take several medications or have an ongoing health condition.

Acai safety at a glance

Acai overall Use caution Acai is generally well tolerated as a food, but supplement quality and high-dose effects are not well studied.
Pregnancy Use caution Acai as a food is likely fine, but concentrated supplements have not been studied in pregnancy, so it is best to use caution.
Breastfeeding Use caution There is not enough safety information on acai supplements during breastfeeding, so caution is advised.

Safety guidance is general; always confirm with your pharmacist or doctor for your situation.

How we checked

Why No Interaction Is Listed for Tofacitinib

Interactions in our licensed database are recorded against drug classes, not just single drug names. Tofacitinib is filed under 4 classifications — including cytochrome p450 2c19 (cyp2c19) substrates, immunosuppressants, cytochrome p450 3a4 (cyp3a4) substrates. Acai has documented interactions with 1 drug class — the most serious being antidiabetes drugs (moderate) — and none of Tofacitinib’s classifications are among them.

Classification data comes from our licensed Natural Medicines dataset. A medication can belong to classes beyond those recorded there, so this describes how the check was run — it is not a safety guarantee.

What Tofacitinib’s Own FDA Label Lists

This is not an interaction — none of it involves Acai. These are the most notable effects listed in the ADVERSE REACTIONS section of Tofacitinib’s own FDA label, for the medication by itself. Knowing the drug’s own list makes it easier to work out the likely cause if something new turns up while you take both. 6 of them are worth acting on rather than waiting out — marked below.

  • Low blood sugar Worth acting on
  • Blood clots Worth acting on
  • A low white blood cell count Worth acting on
  • Liver problems Worth acting on
  • A higher chance of infection Worth acting on
  • An allergic reaction Worth acting on
  • Joint pain mentioned in 29.0% of FDA reports for Tofacitinib
  • Headache mentioned in 7.3% of FDA reports for Tofacitinib
  • Diarrhea
  • A skin rash

From the ADVERSE REACTIONS section of the FDA label for Tofacitinib. Read the label on DailyMed › Where a percentage is shown, it is the share of 185,980 voluntary FDA FAERS reports for Tofacitinib that mention that effect — a reporting proportion, not an incidence rate and not evidence the drug caused it. Acai’s licensed record lists no adverse effects our clinical vocabulary could match, so there is no Acai side to compare — that is a limit of the record, not a statement that Acai has no side effects.

From our pharmacist’s overview on the Tofacitinib page

“Supplements deserve genuine care alongside tofacitinib (Xeljanz, Xeljanz XR), a medicine that deliberately dials down the immune system to control conditions like rheumatoid arthritis and ulcerative colitis. Most of the interactions on this page run through the liver enzymes that clear the drug, meaning an herb can push its blood level…”

About Acai

What is Acai?

Acai
  • Antioxidant support
  • Weight loss
  • Energy and vitality
  • Heart health
  • General wellness ('superfood')

Acai is a nutritious Amazonian berry rich in antioxidants and healthy fats, and it is fine to enjoy as a food. However, strong human evidence is lacking for the bold health claims often attached to acai supplements, including weight loss and disease prevention.

Euterpe oleracea · Arecaceae · Fruit (berry) and its pulp · Juice, frozen pulp, smoothie packs, powder, capsules, tablets

An interaction check asks whether these two react with each other — it does not ask whether Acai is a good idea for someone in your situation. Here is what our Acai record says about it on its own.

Acai: dosing

There is no officially established or standardized dose for acai. Amounts used in studies and products vary widely depending on the form (juice, pulp, powder, or capsule). The best approach is to follow the directions on the product label and not exceed the recommended amount.
