Adenosine and Guaifenesin, Oxtriphylline

Here is the interaction we found between these two, with the evidence behind it.

Minor

METHYLXANTHINES

Why this applies to Guaifenesin, Oxtriphylline

Guaifenesin, Oxtriphylline falls under METHYLXANTHINES — the group Adenosine interacts with.

Methylxanthines are competitive antagonists of adenosine and can block its pharmacologic effects.
The methylxanthines, aminophylline, caffeine, and theophylline, can block the effects of adenosine by acting as competitive antagonists at adenosine cell surface receptors. It is recommended that methylxanthines be avoided for 24 hours prior to cardiac stress tests.
Severity Minor Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the simple version. Brondecon contains oxtriphylline, which your body turns into theophylline, a methylxanthine. Methylxanthines work partly by blocking adenosine's effects at its receptors. So if you take an adenosine supplement (sometimes sold as AMP or ATP), the theophylline can essentially cancel out some of what the adenosine is trying to do, and vice versa. They tug in opposite directions.

The good news: this is a theoretical concern based on how the two work, and it's rated minor. In practice, swallowed adenosine supplements aren't well absorbed, so a big effect is unlikely. The place it really matters is medical: theophylline can interfere with adenosine used during a cardiac stress test, so always tell your doctor what you take before any heart testing.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).
  • BCase-control study.
  • CConsensus.
  • DAnecdotal evidence.

From our pharmacist’s overview on the Guaifenesin, Oxtriphylline page

“Brondecon pairs guaifenesin with oxtriphylline, a theophylline relative that opens the airways, and that second ingredient is why I read this supplement list with real caution. The worst matches are stimulant herbs: ephedra and Sida cordifolia contain ephedrine, which alongside a methylxanthine like oxtriphylline can push heart and ner…”

About Adenosine

What is Adenosine?

Adenosine
  • Energy and cellular function support
  • Athletic performance and muscle energy
  • Circulation support
  • General wellness

Adenosine is a natural building block your body uses for energy and cell signaling, and a prescription injectable version is used by doctors to treat certain fast heart rhythms. As an over-the-counter supplement (often sold as AMP or ATP), there is little strong evidence that swallowing it boosts energy or performance, and quality can vary. Talk to your pharmacist before using adenosine supplements, especially if you have a heart condition.
