Here is the interaction we found between these two, with the evidence behind it.
ORGANIC ANION TRANSPORTER 3 (OAT3) SUBSTRATES
Why this applies to Bempedoic Acid, Ezetimibe
Bempedoic Acid, Ezetimibe falls under ORGANIC ANION TRANSPORTER 3 (OAT3) SUBSTRATES — the group African Wild Potato interacts with.
Some substrates of OAT3 include cephalosporins, famotidine (Pepcid), furosemide (Lasix), hydrochlorothiazide (HCTZ), methotrexate, NSAIDs, probenecid, ranitidine (Zantac).
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for Nexlizet, which combines bempedoic acid and ezetimibe to help lower cholesterol. Your kidneys use a little doorway protein called OAT3 to help clear certain medicines out through your urine. In test-tube studies, African Wild Potato appears to block that doorway. In theory, if that same thing happens in people, bempedoic acid could leave your body a bit slower and build up somewhat higher than usual.
What I want you to know is that this is very early evidence, from lab work only, not from actual patients. So I'm not ringing alarm bells. If levels did creep up, the things to watch for with bempedoic acid are muscle aches or, less commonly, gout flares or joint pain. Before you pair these two, just run it by your pharmacist or doctor so they can keep an eye on things.

From our pharmacist’s overview on the Bempedoic Acid, Ezetimibe page
“Nexlizet pairs bempedoic acid with ezetimibe to lower LDL cholesterol, and since the pill only works if it gets absorbed and holds a steady level, the 21 supplements below deserve careful handling. The single major-rated entry, activated charcoal, can bind the drug in the gut and cut how much reaches the blood, so I would rule that one…”
