Why this applies to Mometasone, formoterol

Mometasone, formoterol falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group African Wild Potato interacts with.

In vitro research shows that African wild potato extract inhibits cytochrome P450 2C9 (CYP2C9). However, this effect has yet to be reported in humans. Until more is known, use with caution. Theoretically, concomitant use of African wild potato with CYP2C9 substrates might increase the risk for adverse effects from these substrates. Some of these drugs include nonsteroidal anti-inflammatory drugs (NSAIDs) such as diclofenac (Cataflam, Voltaren), ibuprofen (Motrin), meloxicam (Mobic), and piroxicam (Feldene); celecoxib (Celebrex); amitriptyline (Elavil); warfarin (Coumadin); glipizide (Glucotrol); losartan (Cozaar); and others.

Here's the honest picture with Zenhale (mometasone plus formoterol). The concern on file is that African wild potato might block a liver enzyme called CYP2C9, which could let certain drugs build up. But Zenhale is an inhaled medicine, and neither mometasone (a steroid) nor formoterol (a long-acting bronchodilator) relies much on CYP2C9. So this particular pairing isn't a strong match, and the worry is only theoretical, coming from lab dishes, not real people.

Neither drug is a prodrug, so nothing here flips the direction. If you ever notice a fast or pounding heartbeat, shakiness, or jitteriness from the formoterol side, mention it. Before adding this supplement, please run it by your pharmacist or doctor first.

Mometasone, formoterol also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

In vitro evidence shows that African wild potato extract can inhibit cytochrome P450 3A4 (CYP3A4) enzyme activity by up to 86%. Theoretically, African wild potato might increase levels of drugs metabolized by CYP3A4. However, in human research, African wild potato does not seem to affect the pharmacokinetics of Efavirenz, a CYP3A4 substrate. Until more is known, use African wild potato cautiously or avoid in patients taking drugs metabolized by CYP3A4. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), ketoconazole (Nizoral), itraconazole (Sporanox), fexofenadine (Allegra), triazolam (Halcion), and numerous others.

Your Zenhale inhaler combines two medicines: mometasone (an inhaled steroid) and formoterol (a long-acting bronchodilator that opens the airways). Both are partly broken down by a liver enzyme called CYP3A4. In a test-tube study, African Wild Potato slowed that enzyme quite a bit. In theory, that could let a little more of these medicines build up in your body.

If that happened, you might notice more steroid-type effects or more formoterol effects like a faster or pounding heartbeat, shaking hands, or feeling jittery. But this is only lab data, and a human study with a similar drug showed no real change, so the risk looks low. Since you breathe these in (not swallow them), even less reaches the liver. Still, it's worth a quick chat with your pharmacist or doctor before combining them.
