Your body relies on a liver enzyme called CYP3A4 to clear bosentan from your system. Lab research hints that American elder may slow this enzyme down a bit. If that happened in real life, bosentan could build up more than usual, which might mean more of its typical side effects, things like headache, flushing, swelling in your legs or ankles, or feeling unusually tired. Bosentan is not a prodrug, so slowing its breakdown means stronger effects, not weaker ones.

Here's the reassuring part: this concern comes only from test-tube studies, not from people, so it's theoretical at this point. Still, since bosentan already needs careful liver monitoring, it's worth mentioning any elderberry product to your pharmacist or doctor before starting it.

American elder is a North American plant whose cooked, ripe berries and flowers are used mainly for colds, flu, and general immune support. The science behind these uses is limited and most studies focus on the closely related European black elder (Sambucus nigra), so benefits are uncertain. Only properly prepared berries and flowers should be used, since raw berries, leaves, bark, and seeds can cause poisoning.

What Is Bosentan Used For?

Brand name: Tracleer

Bosentan is used to treat pulmonary arterial hypertension (PAH) — high blood pressure in the arteries of the lungs. In adults, it helps improve the ability to exercise and slows how quickly the disease gets worse.

In children aged 3 years and older with idiopathic (no known cause) or congenital (present from birth) PAH, bosentan is used to lower resistance in the lung blood vessels, which is expected to improve exercise ability. Tracleer is one brand approved for both adults and pediatric patients with PAH.
