Finasteride, Tadalafil falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group American Elder interacts with.

Theoretically, taking American elder might increase levels of CYP3A4 substrates.

Entadfi combines two medicines: tadalafil (which relaxes blood vessels for symptoms of an enlarged prostate) and finasteride. The tadalafil part is broken down by a liver enzyme called CYP3A4. In test-tube studies, elderberry seems to slow that enzyme down a little, which in theory could let tadalafil build up and lead to stronger effects like headache, flushing, stuffy nose, dizziness, or low blood pressure.

I want to keep this in perspective: this is only lab-based evidence, not something proven in people, so the real-world risk is likely small. Neither drug here is a prodrug, so the direction holds, but I wouldn't lose sleep over it. Still, it's worth a quick mention to your pharmacist or doctor, especially if you take elderberry daily as a concentrated supplement.
