This combination pill (Entadfi) pairs finasteride for an enlarged prostate with tadalafil to help that same condition. Tadalafil in particular is cleared by a liver enzyme called CYP3A4. The trouble is that St. John's wort speeds up that enzyme, so your body breaks tadalafil down faster and leaves less of it working. In plain terms, you may get a weaker effect, meaning the urinary symptoms it's meant to ease (slow stream, frequent trips, getting up at night) might not improve as much as expected.

This is a known, real-world interaction, not just a lab guess, especially with stronger St. John's wort products. Before mixing the two, please check with your pharmacist or doctor so they can guide you.

St. John's wort reduces plasma levels of finasteride in healthy male volunteers due to induction of finasteride metabolism via cytochrome P450 3A4 (CYP3A4). The clinical significance of this interaction is not known.

Here's the gist for Entadfi, which combines finasteride (for an enlarged prostate) and tadalafil (which also helps with urinary symptoms and erections). St. John's wort revs up a liver enzyme called CYP3A4, and that's the same enzyme that breaks both of these medicines down. When that enzyme speeds up, your body clears the drugs faster, so blood levels can drop and the medicine may simply work less well.

In real terms, that could mean less relief from prostate symptoms and a weaker response from the tadalafil side. This is based on pharmacology and a small finasteride study, so the true impact isn't fully known. Still, before combining them, check with your pharmacist or doctor.
