Here's the idea behind this pairing. Your body relies on a liver enzyme called CYP3A4 to clear sirolimus (Fyarro) from your system. In test-tube research, American elder appears to slow that enzyme down. If that happened in real life, sirolimus could build up to higher levels than intended, and since this is a powerful cancer drug with a narrow safety window, even modest increases can matter. That could mean more of its known side effects, like mouth sores, infections, low blood counts, swelling, or high blood sugar and cholesterol.

I want to be honest though: this is only laboratory data, not proven in people, so the real risk is uncertain. Still, given how serious sirolimus is, please check with your oncology team or pharmacist before adding any elderberry product.
