Sirolimus (albumin bound) falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group St. John's Wort interacts with.
St. John's wort induces CYP3A4 enzymes and increases metabolism of CYP3A4 substrates. Clinically significant interactions have been reported with St. John's wort products containing hyperforin 1 mg or more.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
Here's the key thing with Fyarro (albumin-bound sirolimus): your liver and gut clear it using an enzyme called CYP3A4. St. John's wort revs that enzyme up, so your body breaks sirolimus down faster and the levels in your blood drop. Since sirolimus is the active cancer-fighting drug (not a prodrug that needs activating), lower levels mean it may not work as well against the tumor it's meant to control.
That's a real concern, not just theory, so this combination is best avoided. Please don't change anything on your own. Talk with your oncology team or pharmacist before starting or stopping St. John's wort so they can keep your treatment working the way it should.

P-GLYCOPROTEIN SUBSTRATES

St. John's wort induces P-glycoprotein. P-glycoprotein is a carrier mechanism responsible for transporting drugs and other substances across cell membranes. When P-glycoprotein is induced in the gastrointestinal (GI) tract, it can prevent the absorption of some medications. In addition, induction of p-glycoprotein can decrease entry of drugs into the central nervous system (CNS) and decrease access to other sites of action.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
Here's the short version: combining St. John's wort with Fyarro (albumin-bound sirolimus) can work against your treatment. St. John's wort speeds up a transport pump called P-glycoprotein, and it also revs up liver enzymes (CYP3A4) that handle sirolimus. The likely result is lower sirolimus levels in your body, which means it may not fight the tumor as well as it should.
With a cancer medicine like this, losing effectiveness is a serious concern, so this isn't a combination to take lightly. Sirolimus levels are tightly managed for a reason. Please don't start or stop anything on your own. Talk with your oncology team or pharmacist first, since they can check your levels and help you find a safer option.
