Alogliptin, Pioglitazone falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Astaxanthin interacts with.

Theoretically, astaxanthin may decrease levels of drugs metabolized by CYP3A4.
In vitro research shows that astaxanthin induces CYP3A4 enzyme activity in human hepatocytes. This effect has not been reported in humans.

Oseni combines two diabetes medicines: alogliptin and pioglitazone. The pioglitazone part is partly broken down by a liver enzyme called CYP3A4. In a lab dish, astaxanthin seemed to speed that enzyme up, which could in theory clear pioglitazone a little faster and make it slightly less effective at lowering your blood sugar. Neither of these drugs is a prodrug, so faster clearance would mean weaker effect, not stronger.

Here's the reassuring part: this was only seen in test-tube studies, never in actual people, so it's very much theoretical. I wouldn't lose sleep over it. Still, if you start astaxanthin, keep an eye on your blood sugar readings and let your doctor or pharmacist know so they can double-check things.
