Perampanel falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Astaxanthin interacts with.

Theoretically, astaxanthin may decrease levels of drugs metabolized by CYP3A4.
In vitro research shows that astaxanthin induces CYP3A4 enzyme activity in human hepatocytes. This effect has not been reported in humans.

Here's the gist for Fycompa (perampanel). Your body clears this seizure medicine mainly through a liver enzyme called CYP3A4. Lab studies (done in cells, not in people) suggest astaxanthin might speed that enzyme up. If that happened in real life, your body could break down perampanel a little faster, leaving less of it to control seizures. The worry there isn't extra side effects, it's the opposite: a weaker effect and a possible return of breakthrough seizures.

I want to be honest about how thin this evidence is. This was only seen in a test tube and has never been shown in actual patients, so it may not matter at all. Still, because seizure control is too important to gamble with, please loop in your pharmacist or neurologist before adding astaxanthin.

Astaxanthin is a reddish carotenoid pigment with strong antioxidant activity in the lab, and it is widely promoted for skin, eye, heart, and exercise benefits. What Is Perampanel Used For?

Brand name: Fycompa

Perampanel — available as Fycompa and in generic forms — is used to treat partial-onset seizures (seizures that start in one area of the brain) in patients with epilepsy aged 4 years and older. Both the tablet and oral suspension forms carry this indication.

Perampanel tablets and oral suspension are also approved as add-on (adjunctive) therapy for primary generalized tonic-clonic seizures (grand mal seizures involving the whole brain at once) in patients with epilepsy aged 12 years and older. Fycompa oral suspension and tablet share both indications.
