Why this applies to Rifabutin
Rifabutin falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Astaxanthin interacts with.
In vitro research shows that astaxanthin induces CYP3A4 enzyme activity in human hepatocytes. This effect has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist with rifabutin and astaxanthin. Your liver clears rifabutin partly through an enzyme called CYP3A4. In lab studies (using human liver cells in a dish), astaxanthin appeared to speed that enzyme up. If that happened in real people, your body might clear rifabutin a little faster, which could mean lower blood levels and a weaker punch against the infection it's treating.
The important part: this has only been seen in test-tube research, never confirmed in actual patients, so it's quite theoretical and the concern here is small. Still, since rifabutin needs to stay at a steady level to work, it's worth a quick chat with your pharmacist or doctor before you add astaxanthin. Don't change anything on your own.

Rifabutin capsules are used to prevent disseminated MAC disease — a serious bacterial infection caused by Mycobacterium avium complex — in patients with advanced HIV infection.
It is not approved to treat active tuberculosis, and it should not be used alone against active TB. It is also not proven to prevent M. tuberculosis infections.
