Bosutinib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Berberine interacts with.

Theoretically, berberine might increase serum levels of drugs metabolized by CYP3A4.
In vitro research and preliminary clinical research show that berberine moderately inhibits CYP3A4. Theoretically, use of berberine with drugs metabolized by CYP3A4 might increase drug levels and increase the risk of adverse effects.

Here's the gist: your body relies heavily on an enzyme called CYP3A4 to clear bosutinib out of your system. Berberine appears to slow that enzyme down. When the cleanup crew works slower, more bosutinib can linger in your blood, which means you may feel its side effects more strongly. With this drug that could look like worse diarrhea, nausea, belly pain, low blood counts, or strain on your liver.

Bosutinib is already active on its own (not a prodrug), so this points toward higher drug levels, not lower. The data here is mostly lab-based and theoretical, so we can't say exactly how big the effect would be in real life. Since you're treating leukemia, please loop in your oncologist or pharmacist before adding berberine.

About Bosutinib

What Is Bosutinib Used For?

Brand name: Bosulif

Bosutinib is used to treat Philadelphia chromosome-positive chronic myelogenous leukemia (Ph+ CML) — a type of blood cancer. It can be used for newly diagnosed patients or for those whose cancer has stopped responding to, or who cannot tolerate, a prior treatment.

Bosulif (the brand-name product) is approved for adults and children 1 year and older with chronic phase Ph+ CML, and for adults with accelerated phase or blast phase Ph+ CML that is resistant or intolerant to prior therapy. Generic bosutinib tablets are approved for adults only with the same range of disease phases.
