Brexpiprazole falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Berberine interacts with.

Theoretically, berberine might increase serum levels of drugs metabolized by CYP2D6.
In vitro research and preliminary clinical evidence show that berberine can inhibit CYP2D6. Theoretically, use of berberine with drugs metabolized by CYP2D6 might increase drug levels and increase the risk of adverse effects.

Here's the gist for Rexulti (brexpiprazole). Your body relies on a liver enzyme called CYP2D6 to break this medicine down. Berberine appears to slow that enzyme down a bit, so the concern is that brexpiprazole could clear out more slowly and build up in your system. Higher levels could mean more of the usual side effects, like drowsiness, dizziness, restlessness, weight gain, or feeling sluggish.

I'll be honest with you though: this is mostly theoretical, based on lab studies and some early human data, so it may not be a big deal for everyone. Still, since brexpiprazole is one you don't want shifting around unexpectedly, please check with your pharmacist or doctor before adding berberine. Don't change your dose on your own.

Here's the gist for Rexulti (brexpiprazole): your body relies on a liver enzyme called CYP3A4 to break this medicine down and clear it out. Berberine appears to slow that enzyme down a bit. So in theory, pairing the two could let brexpiprazole build up higher than usual in your system.

If levels climb, you might notice more of its common effects, things like daytime drowsiness, restlessness or an inner need to keep moving (akathisia), weight gain, or dizziness. I want to be honest though: this is largely a theoretical concern based on lab work and early studies, not a long track record in real patients. Food amounts you'd get from cooking aren't the issue, but concentrated berberine capsules could be. Please loop in your doctor or pharmacist before adding berberine so they can keep an eye on how you feel.

What Is Brexpiprazole Used For?

Brand name: Rexulti

Brexpiprazole tablets are approved for two uses in adults: as an add-on (adjunctive) treatment alongside antidepressants for major depressive disorder (MDD), and as a treatment for schizophrenia. These uses apply to both the generic brexpiprazole tablets and Rexulti.

Rexulti has one additional approved use: treating agitation associated with dementia due to Alzheimer's disease in adults. Rexulti is also approved for schizophrenia in adolescents ages 13 and older. Note that Rexulti is not approved for dementia-related psychosis without agitation from Alzheimer's disease, and it is not meant to be used on an as-needed basis for Alzheimer's-related agitation.
