Brompheniramine, Dextromethorphan, Phenylephrine falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Berberine interacts with.

Theoretically, berberine might increase serum levels of drugs metabolized by CYP2D6.
In vitro research and preliminary clinical evidence show that berberine can inhibit CYP2D6. Theoretically, use of berberine with drugs metabolized by CYP2D6 might increase drug levels and increase the risk of adverse effects.

Here's the gist with Dimetapp DM. Your body uses an enzyme called CYP2D6 to help break down dextromethorphan (the cough suppressant in this product). Berberine appears to slow that enzyme down a bit. In theory, that could let dextromethorphan build up more than usual, which may bump up side effects like drowsiness, dizziness, or feeling foggy. The brompheniramine (antihistamine) can add to that sleepy feeling too.

The good news is that this is mostly theoretical, based on lab work and only weak human evidence, so for most people taking a short course of cold medicine it's unlikely to cause real trouble. Still, if you use berberine regularly, it's worth a quick chat with your pharmacist before combining them.

Theoretically, berberine may increase serum levels of dextromethorphan.
Preliminary clinical research shows that berberine can inhibit cytochrome P450 2D6 (CYP2D6) activity and reduce the metabolism of dextromethorphan. This may increase the effects and side effects of dextromethorphan.

Here's the gist for your Dimetapp DM combo. The dextromethorphan piece (the cough suppressant) is normally cleared by a liver enzyme called CYP2D6. Early research suggests berberine can slow that enzyme down, so dextromethorphan may hang around longer and build up a bit. It's not a prodrug, so more drug in your system means stronger effects, and that could mean more drowsiness, dizziness, or that foggy, floaty feeling. At high enough levels some people feel nauseated or jittery.

To be fair, this is mostly theoretical and based on limited studies, so for most people it's a mild concern rather than an emergency. Still, since both can cause drowsiness, I'd run it by your pharmacist before pairing them.
