Theoretically, berberine might increase serum levels of drugs metabolized by CYP2D6.
In vitro research and preliminary clinical evidence show that berberine can inhibit CYP2D6. Theoretically, use of berberine with drugs metabolized by CYP2D6 might increase drug levels and increase the risk of adverse effects.

Auvelity is an interesting combo. The dextromethorphan half is actually kept around longer on purpose by the bupropion, which blocks the CYP2D6 enzyme so dextromethorphan can reach your brain. Berberine is also thought to gently slow that same CYP2D6 enzyme, so layering it on top could nudge dextromethorphan levels up even further.

If that happens, you might notice more dizziness, drowsiness, nausea, or a foggy, lightheaded feeling. Keep in mind this is mostly theoretical and based on lab and early human data, not strong proof, so it may not amount to much for everyone. Still, before adding berberine, run it by your pharmacist or prescriber so they can keep an eye on how you feel.

Auvelity combines dextromethorphan with bupropion, and the bupropion is in there on purpose to slow down how fast your body clears the dextromethorphan. Berberine adds another wrinkle. It can mildly block CYP3A4, an enzyme that helps break down dextromethorphan, so in theory levels of the drug could creep up a bit higher.

If that happens, you might notice more of the usual side effects, things like dizziness, drowsiness, nausea, or feeling lightheaded. The good news is this is mostly a theoretical concern based on lab and early studies, so it may not amount to much in real life. Still, it is worth a quick chat with your pharmacist or doctor before adding berberine.

Theoretically, berberine may increase serum levels of dextromethorphan.
Preliminary clinical research shows that berberine can inhibit cytochrome P450 2D6 (CYP2D6) activity and reduce the metabolism of dextromethorphan. This may increase the effects and side effects of dextromethorphan.

Here's the thing with Auvelity: it's a combo pill, and the dextromethorphan half is actually the active ingredient that works on the brain. The bupropion part is included mainly to block a liver enzyme called CYP2D6 so the dextromethorphan sticks around longer. Berberine appears to block that same enzyme. So adding it could pile on top of what the bupropion is already doing, pushing dextromethorphan levels even higher.

In plain terms, you might notice more side effects like dizziness, nausea, drowsiness, or feeling a bit foggy or unsteady. The research here is early and considered theoretical, so I wouldn't panic. But because this combo is carefully dosed, please check with your pharmacist or doctor before starting berberine rather than adjusting anything yourself.
