Desvenlafaxine falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Berberine interacts with.

Theoretically, berberine might increase serum levels of drugs metabolized by CYP2D6.
In vitro research and preliminary clinical evidence show that berberine can inhibit CYP2D6. Theoretically, use of berberine with drugs metabolized by CYP2D6 might increase drug levels and increase the risk of adverse effects.

Here's the gist with Pristiq (desvenlafaxine) and berberine. Your body partly clears this antidepressant using a liver enzyme called CYP2D6. Berberine appears to slow that same enzyme down a bit, so in theory desvenlafaxine could build up a little higher than usual. Desvenlafaxine is the active drug itself, not a prodrug, so more of it in your system means a stronger effect and potentially more side effects like nausea, dizziness, headache, sweating, trouble sleeping, or a faster heartbeat.

I want to be honest though: this is mostly a theoretical concern backed by lab work and some early, lower-quality research, not strong proof. It's worth a quick chat with your pharmacist or doctor before adding berberine, but don't change anything on your own.

Desvenlafaxine extended-release tablets — sold under brand names including Pristiq, Desvenlafaxine ER, Desvenlafaxine Succinate, and others — are approved for the treatment of major depressive disorder (MDD) in adults.

This medication is not approved for use in children or adolescents. Its effectiveness was studied in clinical trials of adult outpatients who met standard diagnostic criteria for MDD.
