Fosaprepitant falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Berberine interacts with.

Theoretically, berberine might increase serum levels of drugs metabolized by CYP3A4.
In vitro research and preliminary clinical research show that berberine moderately inhibits CYP3A4. Theoretically, use of berberine with drugs metabolized by CYP3A4 might increase drug levels and increase the risk of adverse effects.

Here's the thing to know about Emend (fosaprepitant), the IV medicine your team gives to help prevent nausea and vomiting from chemotherapy. Once in your body it becomes aprepitant, which is cleared by a liver enzyme called CYP3A4. Berberine appears to slow that same enzyme down a bit, so in theory the aprepitant could hang around longer and reach higher levels than expected.

If that happened, you might notice more of its usual side effects, like tiredness, hiccups, mild stomach upset, or headache. Honestly, since fosaprepitant is usually a short course given in a clinic, this overlap is often brief. Still, it's worth telling your oncology pharmacist or doctor that you take berberine so they can decide if any timing change is needed. Please don't stop either one on your own.
