Berberine and Ombitasvir, Paritaprevir Ritonavir

Here is the interaction we found between these two, with the evidence behind it.

Moderate

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Why this applies to Ombitasvir, Paritaprevir Ritonavir

Ombitasvir, Paritaprevir Ritonavir falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Berberine interacts with.

Theoretically, berberine might increase serum levels of drugs metabolized by CYP3A4.
In vitro research and preliminary clinical research show that berberine moderately inhibits CYP3A4. Theoretically, use of berberine with drugs metabolized by CYP3A4 might increase drug levels and increase the risk of adverse effects.
Severity Moderate Likelihood Possible Evidence A

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the thing with this hepatitis C combo: your body relies on an enzyme called CYP3A4 to clear part of it, and the ritonavir piece is actually added on purpose to slow that enzyme down so the medicine stays at strong levels. Berberine is also thought to gently block CYP3A4. Stacking another blocker on top could nudge the drug levels a bit higher than intended, which may raise the chance of side effects like nausea, tiredness, itching, or stress on your liver.

To be fair, this is mostly a theoretical concern based on lab and early human data, not a proven problem with this exact medicine. Still, since you're treating a liver infection, I'd hold off on berberine and check with your pharmacist or doctor first.

From our pharmacist’s overview on the Ombitasvir, Paritaprevir Ritonavir page

“Technivie (ombitasvir, paritaprevir, and ritonavir) treats hepatitis C, and because a cure depends on holding steady drug levels for the whole course, this list warrants genuine care. About Ombitasvir, Paritaprevir, and Ritonavir

What Is Ombitasvir, Paritaprevir, and Ritonavir Used For?

Brand name: Technivie

  • The combination of ombitasvir, paritaprevir, and ritonavir is usually used in combination with ribavirin (Copegus, Rebetol), but sometimes it is used alone to treat a certain type of chronic (long-term) hepatitis C infection (swelling of the liver caused by a virus). Ombitasvir is a hepatitis C virus (HCV) NS5A inhibitor. It works by stopping the virus that causes hepatitis C from spreading inside the body. Paritaprevir is a protease inhibitor. It works by decreasing the amount of HCV in the body. Ritonavir is a protease inhibitor. It helps to increase the amount of paritaprevir in the body so that the medication will have a greater effect. It is not known if ombitasvir, paritaprevir, or ritonavir prevent the spread of hepatitis C to other people.
