Bitter Melon and Amphotericin, Tetracycline

Here is the interaction we found between these two, with the evidence behind it.

Moderate

P-GLYCOPROTEIN SUBSTRATES

Why this applies to Amphotericin, Tetracycline

Amphotericin, Tetracycline falls under P-GLYCOPROTEIN SUBSTRATES — the group Bitter Melon interacts with.

Theoretically, bitter melon might increase levels of P-glycoprotein substrates.
Bitter melon might inhibit the p-glycoprotein (P-gp) intestinal pump and increase intracellular levels of P-gp substrates. In vitro research in intestinal cells shows that 1-monopalmitin, a constituent of bitter melon, increases levels of daunomycin, a P-gp substrate. Additionally, drinking bitter melon juice has been associated with a case of acute pancreatitis in a patient who had been taking pazopanib, a P-gp substrate, for 8 years. Researchers theorize that inhibition of P-gp led to increased levels of pazopanib, resulting in pazopanib-induced pancreatitis.

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the short version on combining bitter melon with this tetracycline-containing combination product. Bitter melon may block a gut pump called P-glycoprotein, which can act like a doorman moving certain drugs back out of your body. If that doorman slows down, a little more drug can stay in. In theory, that could nudge tetracycline levels up slightly and make side effects like stomach upset, nausea, or sun sensitivity a bit more likely.

That said, I want to keep this in perspective. This concern comes only from lab and animal studies, not from solid human data with this medicine, so it's quite theoretical. Eating bitter melon as an occasional food is unlikely to matter much, while concentrated supplements or large amounts of juice are more of a question mark. Just give your pharmacist or doctor a heads-up before mixing the two.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.

About Bitter Melon

What is Bitter Melon?

Bitter Melon
  • Blood sugar support
  • Diabetes
  • Digestive complaints
  • Antioxidant source
  • Skin conditions

Bitter melon is a tropical fruit eaten as food and used in traditional medicine, most often for blood sugar control. While some small studies hint it may modestly lower blood sugar, the evidence is limited and inconsistent, so it should not replace proven diabetes treatment. Talk with your pharmacist or doctor before using it, especially if you take diabetes medicines or are pregnant.
