P-GLYCOPROTEIN SUBSTRATES

Why this applies to Glecaprevir, Pibrentasvir

Glecaprevir, Pibrentasvir falls under P-GLYCOPROTEIN SUBSTRATES — the group Bitter Melon interacts with.

Theoretically, bitter melon might increase levels of P-glycoprotein substrates.
Bitter melon might inhibit the p-glycoprotein (P-gp) intestinal pump and increase intracellular levels of P-gp substrates. In vitro research in intestinal cells shows that 1-monopalmitin, a constituent of bitter melon, increases levels of daunomycin, a P-gp substrate. Additionally, drinking bitter melon juice has been associated with a case of acute pancreatitis in a patient who had been taking pazopanib, a P-gp substrate, for 8 years. Researchers theorize that inhibition of P-gp led to increased levels of pazopanib, resulting in pazopanib-induced pancreatitis.

Here's the gist for Mavyret (glecaprevir and pibrentasvir): both of these medicines get pushed out of your intestinal cells by a little pump called P-glycoprotein, or P-gp for short. Bitter melon may slow that pump down. If the pump is working less, a bit more of the drug can get absorbed and build up in your body, which could mean stronger effects and a higher chance of side effects like nausea, headache, tiredness, or itching.

Now, the honest part: this concern comes from lab and test-tube research, not from studies in people taking Mavyret, so it's mostly theoretical. Eating bitter melon as an occasional food is far less likely to matter than concentrated supplement capsules or large amounts of juice. Still, since you're treating hepatitis C, it's worth a quick chat with your pharmacist or doctor before adding it.

What Is Glecaprevir and Pibrentasvir Used For?

Brand name: Mavyret

Mavyret treats acute or chronic hepatitis C virus (HCV) infection caused by genotypes 1, 2, 3, 4, 5, or 6 in adults and children 3 years and older who have no cirrhosis or have compensated cirrhosis (mild liver scarring that is still functioning).

Mavyret is also approved for adults and children 3 years and older with genotype 1 HCV who were previously treated with either an NS5A inhibitor or an NS3/4A protease inhibitor (two earlier classes of hepatitis C drugs) — but not both at the same time.
