Why this applies to Codeine Phosphate, Pseudoephedrine

Codeine Phosphate, Pseudoephedrine falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Black Cohosh interacts with.

Some research suggests that black cohosh might inhibit CYP2D6, but there is conflicting evidence.
Some clinical research suggests that black cohosh might modestly inhibit CYP2D6 and increase levels of drugs metabolized by this enzyme. However, contradictory clinical research shows a specific black cohosh product (Remifemin, Enzymatic Therapy) 40 mg twice daily does not significantly inhibit metabolism of a CYP2D6 substrate in healthy study volunteers. Until more is known, use black cohosh cautiously in patients taking drugs metabolized by CYP2D6.

Here's the wrinkle with codeine: it's a prodrug. Your body has to use an enzyme called CYP2D6 to turn codeine into morphine, which is what actually relieves pain and quiets a cough. Black cohosh might mildly block that same enzyme, and if it does, you'd convert less codeine into its active form. The practical result would be weaker pain or cough relief, not more side effects.

Honestly, the evidence here is shaky and conflicting. One study showed a popular black cohosh product didn't meaningfully change CYP2D6 at all, so this may not matter for you. The pseudoephedrine part isn't really affected. If you notice your codeine isn't working as well, mention it to your pharmacist or doctor before changing anything.
