Here's the concern when black cohosh and methotrexate are taken together: both can be hard on the liver. Methotrexate is well known for occasionally raising liver enzymes or, over time, causing liver damage, which is why your doctor checks your bloodwork regularly. Black cohosh has rare reports linking it to liver injury too. Stacking two things that stress the liver could, in theory, add up to more risk.

I want to be honest about the evidence here: it's mostly anecdotal, not proven, so don't panic. This isn't about how fast methotrexate is cleared, it's simply about giving your liver a double burden. Please check with your pharmacist or doctor before adding black cohosh, and keep up with your routine liver monitoring.

ORGANIC ANION-TRANSPORTING POLYPEPTIDE SUBSTRATES (OATP)

Methotrexate Sodium also falls under ORGANIC ANION-TRANSPORTING POLYPEPTIDE SUBSTRATES (OATP).

Black cohosh may inhibit one form of OATP, OATP2B1, which could reduce the bioavailability and clinical effects of OATP2B1 substrates.
In vitro research shows that black cohosh modestly inhibits OATP2B1. OATPs are expressed in the small intestine and liver and are responsible for the uptake of drugs and other compounds into the body. Inhibition of OATP may reduce the bioavailability of oral drugs that are substrates of OATP.

Here's the gist for combining black cohosh with your methotrexate. Black cohosh appears to gently block a gut transporter called OATP2B1, which is one of the doormen that helps shuttle certain oral medicines from your intestine into your bloodstream. In theory, blocking that doorman could let a little less of your methotrexate get absorbed, which might make it work a bit less well.

Honestly, though, this is rated as a minor concern, and the evidence is only from lab and test-tube studies, not real people. So I wouldn't lose sleep over it. The bigger point with methotrexate is that it's a serious drug where dosing matters, so before adding any new supplement, run it past your pharmacist or doctor first.
