Mirikizumab-mrkz falls under HEPATOTOXIC DRUGS — the group Black Cohosh interacts with.
There is concern that black cohosh might be linked to cases of liver failure and autoimmune hepatitis.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the simple version. Mirikizumab (Omvoh) is an injectable antibody used for ulcerative colitis and Crohn's disease. It isn't broken down by liver enzymes the way pills are, so black cohosh won't change how much of it is in your body. The concern here is different: both this medicine and black cohosh have each been linked, in rare cases, to liver irritation. Stacking two things that can bother the liver may add up.
To be honest, this is a theoretical worry built on scattered case reports, not strong proof. Still, it's reasonable to be cautious. If you take Omvoh and want to try black cohosh, check with your pharmacist or doctor first, and watch for yellowing skin, dark urine, or belly pain. Your care team may already monitor your liver labs.

What Is Mirikizumab-Mrkz Used For?
Brand name: Omvoh
Omvoh (mirikizumab-mrkz) is used to treat moderately to severely active ulcerative colitis in adults — a condition where the large intestine becomes chronically inflamed and ulcerated. It is also used to treat moderately to severely active Crohn's disease in adults, another form of inflammatory bowel disease that can affect any part of the digestive tract.
Both uses are specific to Omvoh administered by injection — first as intravenous (into the vein) infusions to start treatment, then as subcutaneous (under the skin) injections for ongoing maintenance.
