Here's the simple version: Besremi (ropeginterferon alfa-2b) can be hard on your liver on its own, and so can black cohosh in rare cases. This isn't about one speeding up or slowing down the other. It's about two things that may each stress the liver being used at the same time, which could add up. With Besremi, doctors already watch for raised liver enzymes, fatigue, and yellowing of the skin or eyes, so you don't want to stack on another possible liver strain.

To be fair, the worry with black cohosh comes mostly from scattered case reports, not strong studies, so the evidence here is weak. Still, since your liver is already being monitored, please check with your pharmacist or doctor before adding black cohosh, and tell them right away if you notice dark urine, belly pain, or unusual tiredness.

What Is Ropeginterferon alfa-2b Used For?

Brand name: Besremi

BESREMi is approved to treat two blood disorders in adults: polycythemia vera (a condition where the bone marrow makes too many red blood cells) and essential thrombocythemia (a condition where the bone marrow makes too many platelets).

Both conditions are treated with BESREMi given by subcutaneous (under-the-skin) injection. It works to bring blood cell counts back to safer levels and reduce related risks.
