These two interact in 2 distinct ways. Each one is explained separately below.
Why Are There 2 Interactions?
Black Pepper interacts with whole categories of medication. Caffeine , Sodium Benzoate falls into 2 of those categories, so it can interact with Black Pepper in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).
CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
1 of 2Why this applies to Caffeine , Sodium Benzoate
Caffeine , Sodium Benzoate falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Black Pepper interacts with.
In vitro research and pharmacokinetic simulation data suggest that piperine, a constituent of black pepper, as well as the pepper fruit seem to inhibit CYP3A4. This has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for caffeine and sodium benzoate (an injectable caffeine product). The concern on file is that piperine, the active compound in black pepper, may slow down a liver enzyme called CYP3A4. If that enzyme is partly responsible for clearing your medication, more of it could linger in your system, which in theory could nudge up caffeine-type side effects like jitteriness, a racing or pounding heartbeat, trouble sleeping, or feeling wired.
That said, I want to keep this in perspective. This idea comes only from lab and computer-model studies, and it has not been seen in actual people. Caffeine is also handled mostly by a different enzyme (CYP1A2), so any real effect is likely small. Sprinkling pepper on food is no worry. If you take concentrated piperine supplements, just mention it to your pharmacist.

CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES
2 of 2Caffeine , Sodium Benzoate also falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES.
In vitro research suggests that black pepper induces CYP1A2. This has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for caffeine and sodium benzoate, an injectable caffeine product that your liver clears using an enzyme called CYP1A2. The lab idea on file is that black pepper (piperine) might speed up that enzyme, which in theory could break down caffeine a bit faster and slightly soften its effects, like a little less of the alertness or jitteriness you'd expect.
But honestly, I wouldn't lose sleep over this one. This is a theoretical, test-tube finding that has never been shown to matter in real people, and it's rated only minor. The sprinkle of pepper on your dinner certainly won't move the needle. If you take a concentrated piperine supplement regularly, just mention it to your pharmacist or doctor so they can keep an eye on things.

From our pharmacist’s overview on the Caffeine , Sodium Benzoate page
“Caffeine and sodium benzoate is itself a stimulant medication, so the supplements most likely to cause trouble are the ones doing the same job. Seven of the 208 ingredients on file rate major, several of them stimulant herbs such as ephedra that can pile extra blood pressure and heart strain onto a drug already revving the nervous system.”

About Black Pepper
What is Black Pepper?
- Flavoring and culinary use
- Digestive complaints like bloating and indigestion
- Boosting absorption of other supplements
- Antioxidant and anti-inflammatory support
- Cold and congestion remedies
Black pepper is a common kitchen spice that is generally safe in the amounts used in food. Its extract, piperine, is mostly added to supplements to help the body absorb other ingredients (like curcumin from turmeric). Strong human evidence for black pepper treating specific health conditions is limited, so use supplement-strength products with care and check with your pharmacist.
