Drugs with CNS stimulant properties might increase the risk of hypertension and the adverse cardiovascular effects of ephedra.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
Here's the simple version: both ephedra and the caffeine in this medicine are stimulants. Caffeine and sodium benzoate is itself an injectable form of caffeine, so it already revs up your heart and nervous system on its own. Stack ephedra on top of that, and you're doubling down on two stimulants at once.
That combination can push your heart rate and blood pressure higher than either one alone, and may bring on a racing or pounding heartbeat, jitteriness, headache, trouble sleeping, and in serious cases dangerous spikes in blood pressure. Honestly, ephedra is banned in U.S. supplements for good reason, so I wouldn't pair it with anything stimulant-like. Please run this by your pharmacist or doctor before combining them.

Use of ephedra with caffeine or other methylxanthines such as theophylline might increase the risk of stimulatory adverse effects. There is also some evidence that using ephedra with caffeine might increase the risk of serious life-threatening or debilitating adverse effects such as hypertension, myocardial infarction (MI), stroke, seizures, and death.

Here's the simple version: both of these are stimulants, and stacking them piles on the same effects. The injectable caffeine and sodium benzoate product is basically a strong dose of caffeine, while ephedra (ma huang) acts like ephedrine on your heart and nerves. Put them together and you're hitting the gas pedal twice.
That combo can push your heart to race, spike your blood pressure, and trigger jitteriness, trouble sleeping, headache, or even chest pain. In serious cases, ephedra plus caffeine has been linked to dangerous problems like very high blood pressure, irregular heartbeat, seizures, or stroke. The evidence here is mostly from reported cases, but the risk is real enough that I'd steer clear. Please talk with your doctor or pharmacist before combining them.

CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES

Some animal research suggests that ephedra induces CYP1A2 and increases the clearance of CYP1A2 substrates such as caffeine.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the interesting twist with this pairing. Caffeine is cleared from your body mainly by a liver enzyme called CYP1A2. Some animal research suggests ephedra can speed up that enzyme, which would push caffeine out of your system faster. In theory, that means your caffeine could wear off sooner and feel less effective.
But honestly, the bigger concern isn't the metabolism. Both ephedra and caffeine are stimulants, so taking them together can stack up effects like a racing heart, jitteriness, anxiety, trouble sleeping, and rising blood pressure. Ephedra is banned in U.S. supplements for good reason. I'd steer clear of it and chat with your pharmacist or doctor about safer choices.
