Insulin determir falls under ANTIDIABETES DRUGS — the group Black Pepper interacts with.
Animal research shows that piperine, a constituent of black pepper, can reduce blood glucose levels. Monitor blood glucose levels closely. Dose adjustments might be necessary.

Here's the gist with Levemir (insulin detemir) and black pepper. Insulin detemir already lowers your blood sugar, and some animal studies hint that piperine, the active part of black pepper, may nudge blood sugar down a little too. Stacked together, the theory is your sugar could dip lower than expected, leading to hypoglycemia. Watch for shakiness, sweating, dizziness, confusion, or a racing heart.
That said, this is a theoretical concern based mostly on lab and animal data, not strong human studies. The sprinkle of pepper on your dinner is very unlikely to matter. Concentrated piperine supplements are the bigger question mark. If you want to use one regularly, check your numbers more often and loop in your pharmacist or doctor first.
