Here's the gist with sitagliptin (Januvia) and black pepper. Sitagliptin already lowers your blood sugar by boosting natural hormones that kick in when sugar is high. The thought is that concentrated piperine (the active part of black pepper) may nudge blood sugar down a little on its own, so stacking the two could push your levels lower than expected.

I want to be honest about how soft this is: it's based mostly on animal studies, not solid human data, so it's a theoretical concern. The sprinkle of pepper on your dinner won't matter. If you take a concentrated piperine supplement (often paired with turmeric/curcumin), just keep an eye out for low-sugar signs like shakiness, sweating, or dizziness, and loop in your pharmacist or doctor before adding it.

What Is Sitagliptin Used For?

Sitagliptin is used alongside diet and exercise to improve blood sugar control in adults with type 2 diabetes mellitus. It is available as tablets (Januvia, Zituvio, and generic sitagliptin products) and as an oral solution (Brynovin), all approved for this same use.

Sitagliptin is not for type 1 diabetes. It has not been studied in people with a history of pancreatitis, so its safety in that group is unknown.
